Problem: What problem does your solution address?

Until now, "limited resource" becomes the cliche for poor health service for the poor. The same applies to waste management. However, any endeavours that are arranged by the government and private sectors in enhancing the quality of public health could have significant impacts if accompanied by individuals and communities awareness. If organized, community can mobilize their own resources to improve the service they need. We open health access.

Solution: What is the proposed solution?

Garbage Clinical Insurance(GCI) is a micro health funding which can be run by minimum effort within a community and can lead to optimum results. The main principle is organizing community to create sustainable financing from their own resources in order to improve the access and quality of public health program, from promotive, preventive,curative and rehabilitative aspects. On the other side, this becomes an incentive for community to start a proper waste management from household level, thus contributing to improve local sanitation. Garbage Clinical Insurance is empowering every individual to mobilize overlooked resources and take active role in managing health financing.

Founding Story: Share a story about the “Aha!” moment that led you to get started and/or to see the potential for this to succeed.

I believe that health is fundamental human rights. But, in reality many people can't get health access becaused of they don't have enough money and the cost for medication is expensive. Then, we start to think, How could we create health financing model which is permit all of people to get health access. Yeah, Garbage is the best solution, because almost every day every house produced garbage which is not used, so that all citizens can join with our program. Besides, in general, residents must pay a fee to dispose of their garbage, we offer to residents to submit their garbage to us, and they can go for free treatment, counseling in clinic, and get promotive, preventive, and rehabilitative health program. Garbage Clinical Insurance (GCI) is a health insurance program which uses garbage as financing tool. With this program, community give their garbage to the clinic regularly. Afterward, collected garbage is processed in two ways, organic waste is processed to be fertilizer by applying Takakura composting method and Inorganic waste, such as paper, cans, bottles, plastic, is sold to collectors.
Each month the collected garbage reached about 10.000 IDR, which is used to cover health care in a holistic manner, covering the patient's treatment(curative), making health care quality improvement programs (promotive), preventing illness(preventive), and rehabilitative aspects, such as free treatment, inclinic counseling. laboratory check, etc.

Audience: Who have you identified as your customers/recipients and why? How will you get your solution to them or engage them in your initiative?

Garbage Clinical Insurance main stakeholders are scavenger(waste collector), community, and healthworker. The insurance system with garbage gives broad access to the community because almost every day every house produces garbage which is not used, so that all citizens can join the program. The active roles of individuals and communities will determine the success of the health services; however their role must not be separated from health workers’ endeavours to build health awareness and knowledge.

Impact: What is the impact of the work to date and expected impact in the future?

Garbage Clinical Insurance has already replicating in clinics in 5 areas. Member can access clinic health service and health promotiononly by submitting their garbage to the clinic. Garbage Clinical Insurance also distributed 2000 membership cards. In the future Garbage Clinical Insurance aim to replicate in other places to enhance and spread its usefulness in broader zones. Physicians run this program on Monday to Saturday at 7 am to 9 pm. GIC has also recruited 19 subdistrict facilities who expand its access to the clinic health services. In order to increase program sustainability, Human resources in GIC are recruited from all clusters of healthworkers, such as, doctors, midwives, nurses, pharmacists, denstists, and health volunteers. GIC has recruited 88 volunteers, 15 doctors, 12 nurses. All of these programs are paid with garbage from the community makes GIC has sustainibility.

Marketplace: Who else is addressing this problem and how does the proposed solution differ from these approaches?

By far there are a number of groups with similar waste collection initiatives, which trade the waste with an amount of money or goods. There are also a number of insurance in operation. We combine both approaches - exchanging waste into micro insurance and health service. Our distinctive : 1), using garbage as a source of financing. 2), holistic health system, namely health promotion, prevention of illness, treatment for the sick, and rehabilitation after illness. 3), Social enterpreneurship. 4), broad access, micro insurance system with this garbage have broad access to the community.

Scaling the Solution: How do you intend to scale your activities over the next two years (e.g., reach new markets, diversify solutions, etc.)? What will make this possible?

Stages of development in GIC to massive duplication is initiation, stabilization, and independence.

Given that the goals of public health, which in principle is to gather public potentials or resources that exist in the society itself to the efforts for promotive, preventive, curative, and rehabili-tative and to be able to manage the health resources independently, then one of our focus is to establish the independency of the program (community can operate independently without us. Because the main focus of this insurance program is to collect garbage public resources

Financial Sustainability: What is your business model to ensure financial sustainability?

Garbage Clinical Insurance is independently and sustainably maintained by community organization. Financially the program is supported by the community through the trading of waste. Residents’ collected garbage is processed into money towards the"health fund" in 2 ways. Organic garbage is processed to be fertilizer by applying Takakura method and inorganic garbage(such as bottles,plastic,cans,etc) is sold to waste collectors which is contracted.

I founded Indorganization focusing on innovative health product,such as:
1)Telemedicine
Community can get free treatment, clinic counseling, and promotive, preventive, and rehabilitative health programs by telephone. We serve free health consultation to all people.
2)Perinatal maternal Psychiatry
We colaborate with HMC hospital to focus in mental promotion for pregnant woman. As we found 8 from 10 mothers to be to have this problem.
