Our model in San Diego functions as more of a full-service inpatient replacement/alternative for voluntary patients than a step-down facility, but all levels of inpatient alternative care are certainly needed. You probably already are aware of the book edited by Richard Warner (Alternatives to the Hospital for Acute Psychiatric Treatment, published by the American Psychological Association in 1995), but it describes various models around the country. Studies of our model are described in two articles in Psychiatric Services (November 2005, pp. 1379-1386 and March 1999, pp. 401-406, first author Hawthorne for both). The START model was also profiled by Psychiatric Services in the October 2001 issue (pp. 1383-1385) when it received an award. Again, all the best!
