Incentivizing the use of health services is a great idea, and I see the potential for uptake if the smart cards' incentives i.e. discounts are substantial but because like food stamps, this become a identifier or a labeler of sorts, do you have a plan to perhaps remove the identification factor of these cards? Perhaps selling the discounts to the general public at a fee while the sex workers receive them for free?? By having lay consumers also buy into the system may prove to be a more of an incentive for vendors as well as financially supporting your model while also removing the stigma of carrying the smart card.
The most important thing that they have done is, they have got coordinated the male and female sex workers in Mysore, knew their needs, found some solutions for their needs to win their trust and then only they have started the awareness programs among them. It is seen as a significant method to access all sex workers to prevent spreading HIV/AIDS through this community (male and female sex workers).
