Define the innovation

Empowering underserved populations with personal health information is crucial to increasing health care effectiveness. This faith based health initiative develops sustained partnerships with large, inner city congregations to develop individualized Faith Based Health Kiosks which are integrated into the health ministry of the churches to increase the health knowledge of the attendee. The free standing kiosk is customized to the congregation, with an opening prayer and introduction by the lead pastor, and the user enters simple demographic information to direct the interaction. The touch screen kiosk is simple to use, requiring no computer knowledge and is built at an appropriate literacy level for the anticipated users. This kiosk, in partnership with Church, community, state, and national programs, firstly challenges the user to take ownership of their health, and then directs the user to specific things that they can do both preventatively and when health care needs arise. The Kiosk provides customized resources at the church, community, and local level for the user to begin on their "Journey to Wellness"

Context for Disruption:

The main focus of this innovation is a state of the art, computer kiosk based health education program, developed in partnership with large inner city churces, which resides in the health ministry of the church as a reference/resource to increase attendee health knowledge. The primary beneficiaries of this technology are the church body (increased health care knowledge resources), individual parishoners (empowerment thru personal health knowledge and resources), local medical facilities (an informed patient base who will seek health care earlier at a more treatable stage) and local, regional bodies (city and state health department for information dissemination about programs, resources, preventative care etc). This innovation takes advantage of the recurring attendant body of the church, the persuasivness of the pastor, targeted information to the congregation with customized pictures, message, and updates specific for the congregation. The kiosk is effective through its simplicity (touch screen which is intuitive for persons who have never used a computer, voice over for illiterate users, simple messages targeted at the appropriate knowledge level). This kiosk does not require the parishoner or church to have computer or internet services and is readily updated/modified based on church, or local health initiatives or changes in resources. The Kiosk provides an information portal with differing information streams for users with or without health insurance coverage and specifically provides guided references for patients who do not have conventional means to engage health care.

Delivery Model

The Faith Based Health Information Kiosk is placed in a private setting in the individual partner churches. An overview of the Kiosk and the program is described during a service by the pastor with a challenge to use the device. Regular messages are delivered by the partner pastors inquiring about useage rates (the pastors receive updates from the Kiosk team about kiosk use rates and what programs were viewed) This program was only recently distributed, however, the 3 largest congregations in Detroit are current partners (greater than 4000 members per church) and 15 additional churches have requested a Kiosk. We are continuing to measure individual congregation useage rates and will periodically survey the churches about health impact and next steps.

Key Operational Partnerships

Henry Ford Hospital provides the medical specialists who developed the evidence based health modules for the program as well as partnership with the faith based community. Henry Ford Hospital also provides graphic design and computer programming resources and media services to develop this state of the art innovation. Health Alliance Plan provided initial and continued funding for development and distribution of the Kiosk They are also providing health insurance and resource information for the local area. The City of Detroit and State of Michigan have also demonstrated interest in this project and would like to contribute information resources for dissemination and possibly additional funding.

Financial Model

This innovation is not self-funding and is designed to be renenue neutral to the congregations which partner with the faith based kiosk initiative. We have expansion plans which involve modifying the kiosk for commercial, corporate use (Walgreens, CVS, Ford Motor Co, City of Detroit) with expanded medical modules for industrial injuries, prevention, etc, and have designed a pricing structure to allow 15% profit to maintain and expand the Faith Based Kiosk initiative

Effectiveness

All of the initial partner churches involved in this initiative have provided personnel, introduction of the program through sermons, and followup. Approximately 650 people have interacted with the Kiosk to date, and there have been 2 regional news shows and an invitation to the National Baptist Conference. We plan on a broad survey of the congregation at the end of the summer. The partner churches have inculcated this program into their health care ministry and have partnered with us on developing additional health modules for the kiosks.

Which element of the program proved itself most effective?

The most effective component of this program is the partnership with the congregation which ensures embracing this technology and a feeling of ownership by the church. The steadfast approval and empowerment which is afforded by the pastors belief in this innovation has also been essential to its roll out. Finally, the rich multi-media approach, with church specific pictures, video, and message, is a welcoming delivery method for health information

Number of clients in the last year?

The Faith Based Kiosk was delivered to 3 large urban churches 4 months ago, approximately 200-300 parishoners have used the kiosks at each church in the previous 3 months. There are currently 15 additional churches which have expressed strong interest in being included in the initiative. These churches encompass diverse communities including Hispanic, African-American, Muslim, and Jewish origin.

What is the potential demand?

The potential audience for this Faith Based Health Information is enormous encompassing all religions and socioeconomic levels. Additionally, the kiosks are readily modified for a non-faith based audience in commerce and industry

Scaling up Strategy

Our short term priorities are to develop additional modules for health related topics which have been identified by partner churches including elder care, cancer care/screening, and substance abuse and treatment. We are in the process of obtaining funding to increase the number of partner churches and would like to develop kiosks in Spanish and Arabic for our community

Expansion of this plan requires additonal funding for program development, hardware purchase, and additional software development. We have approached local and regional government funding bodies, as well as pursuing corporations for possible sponsorship and/or development of industry targeted health information kiosks which would provide a revenue stream for funding in the non-profit sector

Origin of the Initiative

Henry Ford Health System is a key sponsor for the Faith Based Health Initiative in the City of Detroit through a "Journey to Wellness" program with health screenings, exercise programs, health seminars and fairs, and walks for life. We had just completed a multimedia, educational program for health information for astronauts on the International Space Station and felt that the techniques and technologies could be expanded for use in this format back on Earth through a faith based initiative

What are your two main challenges to finance the growth of your initiative

Lack of availability of funds for church based program to allow expansion of the innovative program to additional churches without resources. 500K would be a significant jumpstart, 1M would allow the development of a robust program with metrics/assessment to allow extra-mural, peer review funding to be realistically pursued.

How did you hear about this contest and what is your main incentive to participate?

This competition was forwarded by an engineering colleague at Ford Motor Company who is working on Disruptive Technologies for the automotive industry. We believe that this approach has widespread applicability across the global community served by your philosophy.

Do you have an annual financial statement?

Henry Ford Hospital has an annual financial statement. The Faith Based Kiosk Initiative was funded through Health Alliance Plan which is a subsidiary of Henry Ford Health System. Henry Ford Health System has an operating budget of 3 Billion dollars per year, and had a profit on operations of over 100 Million dollars in 2006.

Year 1: 250K to develop additional health modules, expand into spanish and arabic, and increase member churches. Begin development of business (industry modules Year 5: 1M total to complete health modules for faith based and commercial based kiosks to allow self-sustaining business model to provide funds for updates and expansion into additional churches
