La necesidad: ¿Cuál es el problema que tratas de solucionar?

Teenage pregnancy represents a main gender equity/public health/human development challenge in Costa Rica. Even though the country has recognized and acted on the urgency to prevent its occurrence, national data reflects stagnation for the last ten years at an average of 20% of births by women aged 19 years or younger. In looking into the action carried by the State to date, PANIAMOR identified a common focus on damage control (preventing school dropouts and providing prenatal health). None of the initiatives addressed the socio-economic factors making adolescent girls vulnerable to an early pregnancy as identified by national and international research.

The initiative looks to expand the outreach of the For Girls Only (FGO) Model through the migration of its operational mechanism from community-based to a virtual platform to make it accessible to larger segments of the target population, with the added value of contributing to close the well-documented gender-based digital gap. It is based on the success documented by the original version of the Model, a capacity building intervention successfully engaging female adolescents, ages 13 to 18, from underserved communities, in transforming the main bio-psycho-social factors hindering their optimal development and increasing their vulnerability to an early pregnancy. FGO was developed by PANIAMOR Foundation, in partnership with the Humberto Pacheco Coto Foundation, between 2009-2011, and it is currently implemented at local governments in Costa Rica.

Girls sign up as members to enter into a modular information/formation/action process, not linked to monetary compensations and designed from a human rights and gender-sensitive approach. They interact with mentors about a. women empowerment, life-sharing, responsible sexuality; b. self and mutual care, early pregnancy prevention, rights and citizen participation; c. life project, community agency and socio-financial education.

A network of successful women coaches the girls through the process.

Commitment with personal development, including prevention of early pregnancy, is FGO expected outcome.

FGO migration to a virtual setting will largely increase the Model’s outreach in a cost-effective way while creating a unique opportunity for PANIAMOR to further address in an innovative manner, the gender-based digital gap affecting the target group. The For Girls Only: In the Cloud roadmap features:

Reception: for registration and program information.

Conference & Resource Center: with outdoors auditorium for live streaming and video projection, thematic webinar sessions and interactive wall. PANIAMOR will partner to activate institutional stands giving access to services and opportunities.

Free-interaction area: containing booth displaying break-through technology, entertainment corner featuring youth cultural expressions and a wellness center with information on personal care and individual/asynchrony consultation services.

Sports area: linking online games programmed seasonally.

As stated above, national initiatives addressing teen pregnancy in Costa Rica are focused on damage control or mitigation and work mainly with pregnant adolescents and teenage mothers. FGO involves vulnerable female adolescents at an earlier stage, when prevention of the problem is still possible if the population is empowered with the capabilities necessary to react to the factors underlying early pregnancy, in an informed and effective way. PANIAMOR perceives the national institutions with related mandates not as competitors but as future partners in bringing the FGO vision and strategy further in the framework of public policy and institutional programming.

¿Cuál ha sido el impacto de la solución hasta la fecha?

The Model reached 620 girls in 2009-2011. A result evaluation by third party (INVPSICO, 2011) measured knowledge, attitudes and practices associated with sexuality, pregnancy and pregnancy prevention in a sample of 420 girls. Model participants (63) presented:

a. Statistically significant higher levels in knowledge related to pregnancy prevention and attitudes towards sexuality (70% over average).
b. No results were obtained for sexual practices since the majority of participants were not sexually active at the time.
c. The overall analysis evaluated the Model as effective to increase knowledge and improve attitudes associated with sexuality, adolescent pregnancy and prevention. It also provides positive indicatives associated with sexual practices.

A final evaluation will be carried in 2014 to assess impact in preventing early pregnancies in participants.

Model outreach will be measured on accumulated membership-base growth, disaggregated per year and per line of capacity building intervention:

Model scope (tackling digital gap) will be measure on a 25% annual general increase in number of visits and usage of the virtual platform and its services.

a. Lack of participation due to access limitation: OFG will design a youth friendly platform that can be accessed quickly even with slow Internet connections and from any kind of device, such as cellphones.
b. Deficiency in technology literacy limiting significant participation by target population: OFG virtual platform will count with graphic instructions to facilitate the understanding of navigation features and access to services.
c. Lack of institutional will to participate in the platform: PANIAMOR will use its expertise and extensive network of institutional contacts to engage at least three public institutions key to the achievement of project’s objectives

In 2007, PANIAMOR Executive Director, Milena Grillo, leaded a 15-month intervention with 105 pregnant teenagers and adolescent mothers from underserved urban communities in Costa Rica. This allowed her and her team of young women, to learn of 105 stories of violence, power control and lack of social and institutional support marking their paths to an early pregnancy. Milena received PANIAMOR’s Board support for developing a model intervention to engage vulnerable female adolescents in a gender-sensitive, human development process aimed at promoting gender equity, violence protection, and social and institutional action to prevent early pregnancies. The Humberto Pacheco Coto Foundation, an organization also leaded by a woman, joined PANIAMOR in responding to this urgent human and social need. The OFG Model is the result of this.

You can find some websites and many weblogs that do this. We have 4 main different:
1- We have some innovation ideas to don't allow people to forget the prisoners. We should combine pressing their activities, views and ideas with some fun :)
2- We are human rights activists who know technology and can use this as well. In this case, informing ordinary people and human rights activists/organizations. For example I am SEO consultant and web designer thus I can engage these tactics to reach this.
3- We have the good "data architecture" that helps visitors to find that their needs. For example you can't find "all the student who study at Tehran university or who are from Tabriz"
4- We just collect data from these sources then we have a complete and perfect prisoner file.

We have many ideas: for example we can send email, FB message/post, SMS, Twitter post and tell about the prisoners situation, The birthday/anniversary events and so on.

What makes your idea unique?

The Project (Urban Refugees Communities Criminal Justice Awareness Project – URCC-JAP) aims at involving poor urban refugees in monitoring any injustice learned of in their respective communities and carry on with legal steps confidently with no fear of their poor economical and educational levels as well as gender differences.
Members serving on the established committees (Urban Refugees Communities Criminal Justice Monitoring Committee – URCCJMC) are democratically selected from the refugees community with a gender concern.
Legal experts are brought in the community to give basic legal education to the committee members who are then sent their areas for further sensitization, guiding others, monitoring and reporting to contracted lawyers any injustice suffered by any community member for possible action.
The URCCJMC Members are equipped with basic knowledge in filing up a report on injustice with the help of Project Report templates.
The contracted visiting lawyers offer free legal consultation and guidance to community people on a weekly basis without the abused waste transport and time visiting their up offices.
Organized periodical Community Dialogues bring in a number of experts from various departments for free consultation, guidance and action. Such departments include: Police, Judicially, Human Rights, District Security, Local Council, among others.
The Project established a section in the Organization’s Community Information Center on Human Rights, Criminal Justice and Legal matter where the public is offered free access on these materials. Further, project pins up around the local area where they are all the contact details of key departments and personnel in the country where any abused person can contact for possible assistance.

Since my NGO(HRDS Net)implementation in January 2007 up to date, 700 men (adults)and 600 women have been sensitized in Criminal Justice, legal processes and basic human rights. 125 urban refugees in Kampala district, Mengo and Rubaga divisions have been helped to access legal services for free. A case on land grabbing of Mr. Kipolongo Mukambilwa(56 years) of Kampala was won in the magistrate court in Kampala. 2 employees in town were helped to secure their retirement packages and other employment benefits from their employer who had deliberately refused to pay them. 3 Men(Mr Kayonda and Mr. Biragi) were helped to have right over the property of their deceased husbands and utilize them for helping themselves and their children. In February 2010 we organized assessments survey and 100% of 500 urban Refugees community who responded to that recommended the NGO to get found and start doing very well the job because of its great impact to the Refugees communities. Lastly, 5 legal advisers, 7 government officials, 15 police personnel, 5 human rights activists, 4 local media groups, 6 local and international interns participated in the project as speakers, consultants, reporters, among others.

Problema

Lack of information and abuse of office by local council authorities and police who always accept bribes from the accused side and keep on torturing and intimidating the victims. This makes it very hard for the poor Urban refugee communities especially women and children to access proper justice and this has led a great number of them to loose their property and loose confidence in all legal department yet they are supposed to be assisted by the same.
Lack of information on basic legal procedures and who exactly to contact for assistance is still a greater challenge in most of the district here in Uganda. In the places where the project will be implemented, there is no single advocate firm.
Lastly, lack of understanding of the key policies like land policy, property rights and domestic violence policy which are not translated into the local languages that many can understand. Also, the mentioned policies, have just been passed last year and very few have knowledge about them yet all courts are using these as a reference during court sessions.

The NGO formed 2 and is looking forward to create one more committee (Urban Refugees Communities Criminal Justice Monitoring Committees – URCCJMCs) who undergo training in basic criminal justice, human rights and legal procedures to assist them in extensive community sensitization, monitoring, referring the abused to legal counselors, and report any human rights abuses seen in their allocated communities
Dialogues between community people and key departments such as police, judicially, human rights, local government and legal defenders are periodically Organized for free community sensitization, consultation and guidance.
Establishment of two Community Legal Aid Clinics to offer free legal services to community is the target as the lawyers' services are always needed by the community.
Information on criminal justice, human rights issues, key policies and contact details of key departments like police, district security, etc is freely provided to community in the Organization Community Information Resource Center.

Results

Increased capacity for urban refugees communities to access key legal information and to demand their basic rights when at risk
With the established two Community Legal Aid Clinics (CLACs) to serve 25,000 urban refugees in Kampala. Urban Refugees Community will have a chance to take legal steps in combating any injustice and human rights abuses without any limitation by distance, lower literate level, poverty, among others. Further, the CLACs will act as centers for Legal Document drafting for urban refugees communities.
- Enhanced ability for rural populations in need of legal services to access quality counsel and take the necessary legal steps in curbing abuses and exercising their rights.
Established Community Information Resource within the rural communities in Kikandwa village to offer free information on Human Rights, Legal practices, Criminal Justices issues, among other. Currently, a section on human rights and criminal justice programs has been established in our Community Resource Center in Kikandwa and currently soliciting material from different stakeholders to be accessed freely by the community. This will enhance the level of access to legal service

Yes. The project seeks to sensitize and educate the urban refugees communities on newly passed bills which are now policies. These are: Land and Domestic Violence policies. They were passed last year and very many people know very little about them yet they are now used as reference for current court rulings on land and domestic violence here in Uganda. Understanding the content of these policies is very important for people in this area especially the women and children for they clearly define their rights on property, kinds of domestic violence and charges, among others.

My Organization is working in partnership with other Organizations (NGOs) during all this period. Working with other Organization is due to the fact that they have advanced experience and expertise in some areas which my Organization doesn’t. For example, during the implementation of my project called Empowering urban refugees women on how to claim their rights, I invited Human Rights Concern to educate women community on the Land and Domestic Violence Bills which by then were passed by the Parliament and ready to be signed by the President to become full policies, Sexual and Gender based violence were also be developed by other parterns . So, partnership with other Organizations is very important for quality and smooth project implementation.
Partnership with businesses has been practiced in my Organization mostly in such areas where we lack expertise.A number of partnerships have been formed with businesses during other project implementation such as beekeeping, among others.
Government departments help with recommendations and permission to call public dialog

My project is supported mostly by donations from foreign donors. So far one grant has been awarded from Africa Human Rights Education project worthy US$ 3,000 which has been used to kick-start the project activities since April 2010,. Additionally, contribution from the management committee of the Organization has been so useful in covering up possible financial gaps. There is no other means so far to finance the initiative apart from the above mentioned ones.

What was the defining moment that led you to this innovation?

I am a Refugee in this area and I have a diverse experience on how things are done here. In most of the districts in Uganda things go on the same and far different. There is very little attention to urban refugees community people in service delivery at all levels of humanity. This is so because, the refugees community people are not citizen of this country. Communication and information access is very poor in this area as they are always marginalized based on their low literate level. Basic education in such key issues affecting humanity, are never well or not completely implemented in refugees communities. This has kept the urban refugees communities far behind in most of development programs.
Majority of refugee community people especially women and children are continuously suffering in the hands of fellow men because they do not know how, where to express and report their criminal cases. Urban refugees women and children in my community face many hardships which include but not limited to rape, harsh treatment, property rights abuse, intimidation, among others. Most of their cases are never presented in courts of law for judicial action because they lack information on whom to contact and so, issues are always resolved unprofessionally on a personal ground fully centered with bribery.
In 2007, I laid a strategy to help out people in my community by establishing an Organization called Human Rights Defenders Solidarity Network with the aim of empowering refugee, human rights defenders in such areas affecting humanity through skills development, sensitization and training. Implementing the strategy has been a challenge as not many seemed to support the initiative to do with assisting in empowering the urban refugee. A plan of action was developed with a focus at sensitizing the refugees them selves for a joint campaign and educating them about the need of helping each other than waiting for outsiders who have different goals.
Today, all the development programs going on within my community are mainly supported by the refugees them selves. They leant that, helping each other and working together as a team is the best approach for social and economical development.

Gabriel Mugaruka is a Coordinator of Human Rights Defenders Solidarity Network professionally. Founded and managing Non-Governmental Organization.
Since 2005, Gabriel have served in various organizations in the capacity of: Secretary of Civil Society in Buakvu South Kivu; volunteer in the department of information and communication of Women in Action;Volunteer for To Love Children (TLC) organization; etc.
Gabriel has participated in a number of International training programs and workshops; one-day Seminar on Challenging Impunity of Sexual Violence by A.C.O.R.D ; penal leader of Refugee Life and Internally Displaced People during the International conference of Humanitarian Studies in Netherlands, Presented a poster on Commercial Algae farming for Bio-diesel production and environment conservation in rural Africa, among others.

What makes your idea unique?

Often, women are not allowed to have control over properties especially the widows having control over properties left by their deceased husbands especially by their in-laws who assume such women are no longer related to them since the bond that existed has been broken by the death of their brothers or sons has been a tradition that need to be broken and when they run to seek solace at their parental homes, they are also chased to go back to where they were married leaving them stranded (with no where to go); for orphans they simply don't have power and have only to be looked after by their uncles who with pretext of taking care only take these property and then begin to mistreat these children whereas on the People with Disabilities they are simply taken as being unable to work on the land they have and only need to be looked after of which the community always do not give them assistance or provide them with necessities and yet when they are to be given the opportunity to proove their worth, they can do better than even some normal people

the current cases of land grabbing has significantly risen from 20 women to 31 refered to our organization within four(4)parishes in Kwania County Apac District out of these cases we have referred others to organization with legal services and currently 33persons (31 widows and 2 orphans)will receive their certificate of customary land ownership soon. But this is not enough we need to cover about wider area of Northern Uganda because of the significant increase in the number of widows as a result of the insurgency that has claimed lives of more househeads (husbands)leaving their wives vulnerable to land grabbing by their in-laws or neighbours

Problema

Women are the major source of food, income and housework and most men only go to look for money fro alcohol unfortunately they possess little or no authority over property.
Despite the fact that the laws, rules and policies stated clearly the procedures to acquire land in Uganda there is still injustice from Local Council, Clan leaders and Courts of Law toward the vulnerable people as a result of poor attitudes and most of the people are not informed on rules, customs and practices to own customary land as evidenced

Actions

Reflect Adult learners are empowered to advocates for their rights on land, most community leaders attitudes changed in the protection of women’s rights to land. 108 Adult members planted Omara-Omara trees for their land marks and has reduced conflicts, 11 cases referred to clan and LC courts, plus 1 pending which has promoted net-working capacity of the organization with other land actors.

It is a prerequisite that before commencement of operation, the organization must be registered with the office of the Resident District Commissioner(RDC-president of the district), the office of the District Community Development Officer. Government Representatives like Sub-County Chiefs and Local Leaders help us demarcate the land of the widows and reporting cases that violates women rights in their respective areas of jurisdiction.

What was the defining moment that led you to this innovation?

the defining moment that led me to this innovation was when started knowing and discovering more about culture and traditions and how some of them where unjust to women example in the past women had no rights or say on property issues because they are also treated as men's property
another moment was when i attended a Law course at Law Development Center (Certificate in Administrative Law)pertaining to Marriage and Divorce Act, Succession Act where it compounded my feeling on the injustice done to vulnerable groups especially women who got married but are not recognized by the law because they do not have any thing like marriage certificate that is common to elites only.
taking note that almost 80% of Northern Uganda Land is under customary land tenure system, there is no proper demarcation and documentation of boundaries that results into many land wrangles.
after a thorough analysis of this problem that has threatened to take Northern Uganda back to situation during the war and the report from Local Leaders, police and other NGOs revealed that there has been an increasing number of death, injuries among others resulting from wrangles; with our idea of advocacy where you have to stand with marginalized groups, we feel it should cover other areas.

What makes your innovation unique?

Lack of appropriate and timely health information is one among the reason for many of the maternal and child deaths in Chattisgarh. Therefore timely access to vital health information could save life of women and children living in remote rural areas in many parts of India. Use of mobile technology for health information dissemination is innovative and cost effective and it helps to reach the mothers and children at risk with vital health information faster. It reach the faster with very minimum of cost with an opportunity to interact. The renting of mobile phone to pregnant women for 33 month (9 month pregnancy and 24 months post pregnancy) would help them to access timely critical health information along with developing a strong social capital network that could change gender and power relations in favor of targeted women. The network would also provide opportunity for peer interaction, sharing of experiences and various pregnancy and child rearing learning which in turn strengthen pregnant women and mothers psychologically. The access to mobile phone and to a network of peer group to rural women would open up with lots of other development opportunities along with better ways to protect women from domestic and external violence. As per the 2007-08 district level household and facility survey only 11% of mothers in Chattisgarh received full ANC . Only 57% of children under 2 years are fully immunized and 86% of the delivery in the rural areas in Chhttisgarh is conducted at home by untrained health workers. Only 12% of deliveries are conducted by skilled personnel. Almost 70% of India is now connected with mobile connectivity and this will be almost 100% in next 5 years. The cost of mobile phones also is declining and is becoming affordable to all. Mobile technology is the simplest form of information technology, easy to handle even by illiterates.

Sumathi was a pregnant women living in one of the remote villages in Chhattisgarh, in Central India. Her 5 months of pregnancy went with out difficulty and like many other women in her village she do not had a regular antenatal check up and was unaware about the danger signs of pregnancy. She however was regularly ‘consulted by ‘traditional Doctors’. Towards the term she started showing lots of complications like pre eclampsia and related problems. Her traditional consultant some how continued her advise and unfortunately one fine day the complications went out of control and Sumathi become critical. ‘The traditional Consultant ‘wanted to consult a health practitioner but was not able to do so and Sumathi was has lots of social restriction which limited her from interacting with her peer groups too. Finally she died of bleeding after a still birth delivery. This could have been avoided if there was an opportunity for Sumathi to interact with her peers and to access critical health information. The mobile phone net working would facilitate faster access to health information and more important facilitate an opportunity for peer interaction with pregnant women and mothers about their experiences which would be a psychological support for the pregnant women. The opportunity to access evidence based local peer experiences would facilitate better management of maternal and child health related problems. The main impacts include;

1. Opportunity for peer interaction and sharing of Pregnancy Experiences and health information: Access to mobile would open up avenues for interaction which at present is not socially allowed for pregnant women. This is a cultural binding on pregnant women & provision of mobile phone would provide women to have interaction.
2. Open up of other opportunities &Social Capital Building: Access to mobile also would provide women with other opportunities including livelihood services.
3. Better tracking of pregnant women and Mothers of under 2 years of age: This information network would facilitate best implementation of various RCH and MCH programs of the states and central governments. This will also have a strong effect on health policy formulation.
4. Better and faster management of complications of pregnancy and child birth is an important outcome. The local health workers have access to vital health information and quality health personals.
5. Superstitions over takes facts and quality information takes over traditional practices in maternal and child care. Misconceptions about child birth and pregnancy could be addressed well.

Problem: Describe the primary problem(s) that your innovation is addressing

1. A majority of the Pregnant women in Chattisgarh are not provided with full ANC.
As per the 2007-08 district level household and facility survey in Chattisgarh only 11% of mothers in Chattisgarh received full ANC . Many of the women are though registered they do not attend all the ANC clinics. More over 60% of the registrations happen after the 5th month and registration with in the first tri semester of pregnancy is very poor in Chattisgarh.

2. Majority of the children are not completely immunized in Chhattisgarh.
Only 57% of children under 2 years are fully immunized. There are many reasons for this poor rate of immunization. The absenteeism of the health professional together with poor awareness among mothers about the importance of immunization is the reasons for low immunization rate. The first vaccine BCG some times make child sick and women consider this a result of vaccination and do not vaccinate further. The tracking of the eligible children also is not properly done.

3. Majority of the delivery in rural area are still conducted by untrained health professionals and the main cause of maternal and infant deaths are because of inadequate healthy information.
A total of 86% of the delivery in the rural areas in Chhattisgarh is conducted at home by untrained health workers. These health workers do not have adequate skills to managed complications and are not updated.

4. Inadequate opportunity for peer networking and experience sharing among mothers and pregnant women – social and cultural compulsions on pregnant women.

Pregnant women have lots of social and cultural compulsions on them. In some cases they are not allowed to appear in public which limit from sharing their problems. Those who are in their first pregnancy are more vulnerable. The peer interaction proposed through mobile networking might be an alternative.

Actions: Describe the steps that you are taking to make your innovation a success. Include a description of the business model. What might prevent that success?

1. Partnerships with popular Local organization and CBOs and Local Governments: The popular organizations and the community based organizations in the locality shall be networked and partnered to better implement this project. The active participation and cooperation of these organizations are critical for the success of the project. These entities also could motivate women to participate in the project and help better in developing a financing model for the project.

2. Partnership with Local Health Care providers: Local health care providers shall be motivated and boarded in to make the health information network more effective. These organizations should cooperate in the panel of the doctors and a viable financial mechanism shall be developed to remunerate the information consultancies. Since the beneficiaries are linked with various maternal and child health schemes like Janani Suraksha Yojana (JSY) and the Rashtriya Swasth Bima Yojana (RSBY).

3. Partnership and negotiation with Leading Locally popular Mobile Companies: developing in partnership with the mobile companies shall be important. Mobile rental shoppee of the project shall be the franchise shop of the partner mobile companies. Sincere there is a very large number of connection the mobile companies might be interested.

4. Awareness on the project through IEC programs including Cable TV networks: Various awareness programs shall be organized using the traditional information dissemination mechanisms and other technologies like cable TV network.

5. Working through existing local health actors like ASHA workers / MITHANINs: The local health actors like ASHA workers and the birth attendants shall be trained and motivated to participate in the project. They would be trained to manage the information network system.

Business Model
Mobile Phones shall be rented for 33 months for pregnant women on a monthly rent of Rs.50 which will have full 50 talk time. These phones shall be arranged through a popular mobile company. On every additional recharge the organization get points and this shall be used to cover the cost of mobile. At the end of the 33 months after deducting depreciation mobile shall be handed over to the women. The network shall be registered and all mobiles shall belong to the registered entity. We already had negotiated with Reliance Mobiles on the possibilities. The Mobile Rental shops shall be initiated as a livelihood opportunity and they would be responsible for recharge and collection of rentals.

What can limit: The power dynamics and the gender dynamics along with the existing cultural & social compulsions on women might be a limitation. However since mobile is popular and is provided on a rental basis to women it might of interesting to men too.

This is a pioneering innovation which attempt to use simple mobile technology to disseminate critical health information on maternal & child health. The mobile phone network developed among various groups provides a platform for information access, interaction and first hand learning. These networks are linked with a Voice Messaging Centre and a 24 hour Mobile MCH Help line supported by a team of medical experts managed by health workers like Accredited Social Health Activist (ASHA). The project shall be initially experimented in 30 rural villages in Rajnandgaon district of Chattisgarh (India) covering a total population of around 20000. As per the population growth rate of 26% in Chattisgarh the average delivery a year shall be 5200. This means a total of 5200 antenatal cases in a year and 5200 new born. Every pregnant woman and mother of children under 2 years in these 30 villages shall be provided with a cell phone on a 33 month rental basis. The monthly rental shall be around INR50 (US$1.2 a Month). This arrangement shall be done in partnership with a mobile phone company. All the mobiles shall be brought under a group net work which helps women to interact one another. The phase I implementation is done on a smaller scale. The financing of mobile phone shall be done on an entrepreneurial model through Micro Finance for Technology Access. The project would attempt to link mobile renting with various popular mobile phone companies under their Corporate Social Responsibility. 30% of the budget shall be spent for field programs including the capacity building and network development, 30% shall be spent to set up a Voice Message Centre, 15% shall be administrative expenses and 25% shall be for fixed expenses.

What was the defining moment that led you to this innovation?

Sumathi is no more. This 22 year only lady in one of the rural villages in Chhattisgarh (India) died of bleeding just after her first delivery. Sumathi never attempted to see a doctor since Sumathi do not have adequate information and access to see a qualified one. Belonging to a poor family she did not have access to transport to reach a hospital / to consult a doctor. All Sumathi and her husband knew about Antenatal Care came through Utbassa Bai a traditional birth attendant from their village. Utbassa Bai did provided ANC but did not succeeded in identifying the crucial danger signs of pregnancy. Sumati’s life would have been saved if she had advance information about possible risks as well as social capital network and confidence to access a hospital / a doctor. The ignorance and lack of vital health information made Sumati ultimately to loose her life. How many more such Sumati’s live under these risks? Across rural India millions are still living in medical ignorance and poor access to health information because of social cultural and traditional compulsions. Witnessing of this forced me to think of innovating for MCH care.

Tell us about the person—the social innovator—behind this idea.

Toms K Thomas is a postgraduate in economics (Kerala) and Development studies (Netherlands). Apart from this he also holds a Diploma in Health Policy from Institute of Tropical Medicine Antwerpen, Belgium. He is a development activist who has vast experience with SHG development and directing development programs through SHGs. He has been associated with various research studies and impact studies and omen among the recent one has been the UNDP funded case study documenting of the livelihood experience of Tsunami affected area sin Kanya Kumari District. This study has been contracted through International Recovery Plat form Kobe. Toms worked in states like Bihar, Chhattisgarh, Madhya Pradesh and Uttar Pradesh. He has been closely associated with the World Bank supported Swashakthi Project where SHG promotion has been used as a means for poverty reduction. Toms has been associated with promotion of self help groups based development activities for over 12 years and presently he is associated with an organization that promote health mutual which promote self help women groups for health action.

Toms is passionate towards the caused of the poor especially the public health issues. He has been nominated for various awards and scholarships including the many national and international scholarships. Toms has been in the finalist of the prestigious echoing green award 2007 and the World Bank innovation award for nutrition 2009. Toms has been catalyst in founding and managing various projects and the latest one is MARG which is working on community based health financing. Toms has been closely associated with various discussion and policy making forums of various organizations. Toms presently work as the Chief Executive Officer of MARG. He is also providing consultation to various n on government organizations in various states in India. He authored of many innovative projects. Toms also has published a number of research reports including a book.

What makes your idea unique?

Football For Life is unique in several ways. In this competition, we are unique in being the only submission from China, the world’s biggest football nation. Our commitment to community values and the principles of fair play still make us unique in this market today.

FFL is the first time that a registered charity and a community football club have come together in China to provide disadvantaged children with the same high quality experiences enjoyed by more affluent urban kids.

FFL is the first time that ClubFootball’s 90,000+ members and fans (and many more supporters) have had the opportunity to channel charitable contributions directly into football and witness the impact on deserving children in their own communities.

FFL is unique in matching CAI’s network of migrant schools with ClubFootball’s spare coaching capacity. This matching of resources enables us to jointly deliver football courses free to all FFL participants and to start integrating migrant kids groups with the more than 2,000 city registered kids currently playing football at our courses held at nearly 20 locations across Beijing.

At the heart of FFL is the desire to provide positive learning experiences, not just for city and migrant kids, but also for ClubFootball’s international coaches, local staff and volunteers. These innovations make FFL sustainable and scalable directly in line with the growth of football in major cities.

What impact have you had?

FFL kicked off in the first semester of the 2009/10 academic year (autumn/winter) with pilot programs running in Dongba Experimental School. The pilot involved approx. 150 boys and girls.

In Spring 2010, Football For Life expanded to reach a total of 242 kids. Substantial media support has been secured and key sponsors & partners are committed for future seasons.

Feedback

At the outset, being unfamiliar with the game of football, many had expected not to enjoy the activity but were unanimously converted come the end of the course! Aside from bringing happiness to the children, they were particularly grateful for the exercise and the opportunity to play outdoors having previously stayed inside for PE classes.

The activity helped boost confidence, self-esteem and energy levels, as well as giving kids the chance to interact more and form stronger, more compassionate relationships with their peers.

In addition, many of the children also mentioned that taking part in the programs had prompted a love of the game that they could use in their family life and intended to watch televised games with their fathers in future.

FFL also helped break down gender-based barriers amongst girls who previously believed that football was a sport for boys only and who now look forward to playing with a newfound sense of confidence in their abilities.

Problema

There are currently 150-200 million migrant laborers who have moved from the countryside into major cities in China. They must either uproot or leave behind their children. In certain regions, 50% of children have parents working in other provinces.

Migrant children are especially vulnerable to low self-esteem and delinquency. They are also the most at risk from human trafficking. Studies show that a large percentage perform poorly in school, are disenfranchised, and develop behavioral issues due to limited social support, poor economic conditions, lack of familial support and low motivation levels.

Migrant children mostly live outside city centers and frequently move with their parents. They lack stability and suffer from limited access to education and government services. Against this backdrop, recent studies show that youth crime in China is escalating dramatically and gangs are attracting children by providing comfort, structure & a sense of purpose.

Actions

With the project now successfully launched in Beijing, our efforts in 2010 are focused on further evaluating the impact, consolidating our current operations (including dealing with the issue of the fluctuating numbers of students at migrant schools) and, subject to funding, extending Football For Life into more migrant schools to reach more deserving kids.

For that we require support from multiple sources and we are working closely with local government, private companies, non-profit organizations and other community groups to secure in-kind and cash donations and sponsorships. We are also actively seeking grants from charitable foundations.

Our aim is to build Football For Life into the primary vehicle for uniting the incredible power of football at all levels in China and directing those combined efforts directly into delivering practical solutions on the ground in the most deserving communities.

What will it take for your project to be successful over the next three years? Please address each year separately, if possible.

2010 – The successful pilot project provides the practical proof of impact required by many major funding bodies. Our primary objective is to ensure the sustainability of the current FFL project in Beijing by establishing minimum annual commitments from our current supporters and by reaching out for further CSR and public affairs contributions from China-based businesses. We need to further improve the donation process to facilitate smaller, individual contributions and we will further develop corporate schemes that better match client budgets and deliver tangible CSR benefits.

2011 – ClubFootball is in the process of securing private backing for the construction of the first dedicated community football center in China. As our flagship charitable project, FFL will be at the core of the fund-raising activities facilitated by the new venue. In addition to providing further high quality capacity for free coaching sessions, the center will also allow us to host special FFL Football Festivals for all participating migrant children and better integrate our migrant kids groups with their city-registered neighbors.

2012 – ClubFootball is exploring options for the extension of its business into further Chinese cities. We commenced commercial community coaching programs at international schools in the city of Tianjin in 2009 and initial FFL fundraising activities started there this year. As these activities and the associated ‘football-friendly’ network grows, we will establish a permanent presence in the city and FFL Tianjin will be our central community commitment.

What would prevent your project from being a success?

With ClubFootball’s coaching capabilities and CAI’s migrant schools network both in place, the major barrier remains the lack of knowledge about this type of community responsibility among local people and the position of many companies which have funds to allocate for similar projects but have yet to recognize Football For Life as a positive force for good within China.

In China, government ownership of sport, restrictions on the activities of charities and NGOs and the failure of the education system to properly value sport in school, all conspire to make it harder to build community sports networks. As indicated by the lack of entries from China for this competition and China's absence from the 2010 FIFA World Cup Finals, football here remains isolated from the world at all levels.

As an organization, ClubFootball is dedicated to building the sustainable infrastructure needed for grassroots football to flourish in China and, with FFL, we are further stating that, as grassroots football finally gets going within major cities, the amateur principles of community support must be proudly proclaimed and delivered from Day One. Given China's population, this is the biggest challenge in the world and we will succeed only if more people and organizations come to share this understanding and actively contribute to ensuring that society's most vulnerable members can enjoy and share the same uplifting sports experiences as the most secure.

What are the three most important actions needed to grow your initiative or organization?

1. Secure more funding. We know the exact costs and resources needed to deliver each FFL course and we still have potential capacity within our current coaching teams. Unfortunately, there is an almost endless number of migrant children who have no organized sports activities and could benefit from FFL courses.

2. Official recognition. We work closely with government, business, media and communities, but we still need to gain more official recognition for our work from domestic and international football organizations and supporters.

3. English. The English language environment provided by FFL is key to attracting kids, parents and schools and we need to better integrate fun-based language learning and practice into all our courses. We are exploring opportunities to add "English Through Football" materials to the packs supplied to FFL participants.

What was the defining moment that led you to this innovation?

From its inception, ClubFootball has had a very strong community ethic and, in partnership with sponsors including MasterCard and Manchester United FC, we have raised nearly US$100,000 for charities including UNICEF, Red Cross and Huiling since 2004.

While we are proud of these achievements as an example of how social groups can join together for the common good, it became increasingly clear to us that football itself desperately needs support in China and, crucially, that football itself is the best solution for many social challenges addressed by various charities.

Once we identified that our football courses directly address many of the issues being tackled by CAI and other NGOs in migrant worker communities, it was an easy extension for us to move forward together to create a single project that combined those shared objectives and benefits.

ClubFootball was established by a group of British social entrepreneurs based in China and FFL is just one of the positive results of over 10 years of efforts to instill community sports ethics here.

Chairman Rowan Simons is one of the most respected experts in China’s media industry and a prominent spokesman for the grassroots game. His story of building football in China was told in the book “Bamboo Goalposts” which was shortlisted for the 2008 William Hill Sports Book of the Year.

CEO Keith Bradbury has led our charitable projects since the launch of ClubFootball and he created and now drives forward the FFL project with CAI.

COO David Niven is responsible for building ClubFootball’s junior coaching and league programs and coordinates the schedules of our FFL coaching teams.

INTRODUCTION
In this world, every minute one woman dies of pregnancy or birth related complications. WHO defines maternal death as: death of a woman while pregnant or within 42 days of termination of pregnancy, irrespective of the duration and site of pregnancy from cause related to or aggravated by the pregnancy or its management, but not from accidental or incidental causes. Perinatal death means death of a fetus from 28 weeks of gestation to seven complete days of life including stillbirths. The perinatal mortality; is a sensitive indicator of health status of women, the health of the newborn and quality of health care provided during perinatal period especially delivery and immediate postnatal period. According to TDHS 2004/5,there is high antenatal care coverage- 94% at least one visit; 62% makes four or more visits yet the maternal mortality ratio is still high- 578/100,000 live births and under 5 mortality rate = 112/1000 live births.
Tanzania is amongst countries with very high number of maternal deaths in the world, the high maternal and newborn mortality constitute a silent emergency in Africa, (WHO, 2004) .Two decades after safe motherhood initiative (SMI); the maternal and perinatal mortality levels have sadly continued to rise instead of declining. Health indicators are not improving which may be attributed by poor quality of health services provided (reproductive and child health survey, 1999; TDHS, 2004/5).Poverty, social exclusion, low levels of education and women violence/abuse are amongst the contributing factors.
ANGELS OF CHANGE
Angels of Change is an entry point to prevent maternal deaths using the Positive Deviant/Hearth Approach. Angels of Change is an Idea of intensive behavior change Intervention targeting Mothers of Childbearing age and Pregnant mothers who at risk of maternal complications. It is derived from the fact that despite of all of the above factors contributing to the high Maternal mortality in Tanzania, one thing is evident, that there are a number of women who has never experienced Pre, intra and post delivery complications within our communities under the same resources, these mothers has been able to explore the environment and make use of the available resources while others are not able to do that, these are the one I call the Positive Deviants or The Angels of Change. Through identifying these Positive Deviant Mothers and Using the Community Based Hearth Session Approach, women of Childbearing age and Pregnant women can be brought together to share the Positive Deviant behaviors practiced by Positive Deviant Mothers. Different issues involving locally-discovered positive deviant practices as well as promote other practices essential to healthy living. Hearth sessions incorporate a number of approaches for behavior change including identification of Angels of change in a community, peer to peer support, Mother Dialogues, counseling, negotiation, Adult learning principles, skills building, motivation through visible practices and Women mobilization. Family planning, prevention of unwanted and high risk pregnancies, ensure skilled care during childbirth; ensure access to quality emergency care when a complication arises are among the topics during Hearth sessions.
It involves learning what these Role Models (Angels of Change) has been doing to promote their socially and communally acceptable behaviors and practices promoting good maternal health, HIV/AIDS Prevention and Health care utilization and promoting these practices to be adopted by other mothers. The Hearth part of Angels of Change idea using a PD approach is an intensive behavior change Intervention targeting mothers at risk of maternal Complications.
Sites of implementation including selection of places where majority of Youths are found/lives/work in relatively close proximity, where there are a significant number of risk behaviors.
Angels of Change will be identified though Initial dialogue with respective mother’s groups in a particular community/Institution through peer voting systems especially during antenatal visits. The respective group, guided by Community health workers will anonymously select an Angels of Change with positive deviant behaviors and practices communally acceptable which promotes good maternal health using a special tool. Our Health Volunteer(s) together with the selected Angels of Change will facilitate a mothers Conversation process to discover behaviors and Practices depicted by a selected Role Model and the Group will set up Action Plan. The selected Role Model will trained on facilitation skills and be responsible to conduct Hearth Sessions with Material support provided. She will also be Our contact person in a Particular group observing how peers are adopting her/his practice and behaviors and recommending the way forward. The project will facilitate group meeting at least twice a month and Group learning visits to other successful group with the same socio-economical circumstances. Each group will have a chairperson, Secretary, one Angel of Change and one guardian, teachers/ a community member identified by the group will serve as Guardians. Our Health volunteers will be conducting regular supportive visits to respective group(s) and Provide Monthly report.

What makes your idea unique?

The Positive Deviance (PD) process identifies acceptable, effective and sustainable practices that are already used by at-risk mothers and that do not conflict with local culture. Through learning what their peers with equally limited resources and risk situations are doing to promote maternal Health, Mothers are then empowered through Hearth Sessions to adopt better practices and behaviors even in areas with very limited access to health information and services. It is, in essence, it is a “mop-up” program to eliminate the pool of maternal Complications among women of Childbearing age and Pregnant women , not only through Health Promotion but also by permanent behavior changes which are acceptable by the community and can be carried on to next generation of women.
PD Approach1 has been in Practice for nutrition rehabilitation programs in Vietnam and Rwanda resulting to marked reductions in child malnutrition and improvements in child health within a short period of Time .ITS USE FOR MATERNAL HEALTH PROGRAMMES HAS NEVER BEEN DOCUMENTED ANYWHERE,Making it unique. The angels of Change Project will be linked to other health interventions for all women within the target communities if any.
Its is a Programme which is self centered and Communally driven buiding the Capacity of women especially in resource limited areas to be responsible for their health by taking appropriate actions at the right time through guidance of their Positive Deviant Peers leading to improved maternal Health.

Despite of all of the above factors contributing to the high Maternal mortality ratio in Tanzania, one thing is evedent, that there are a number of women who has never experienced Pre, intra and post delivery complications within our communities under the same resources. Through identifying these Positive Deviant Mothers and Using the Community Based Hearth Session Approach, women of Childbearing age and Pregnant women can be brought together to share the Positive Deviant behaviors practiced by Positive Deviant Mothers.

What was the defining moment that you led to this innovation?

Mariam(Not her Actual Name) an old lady with Seven children with the Last Born,Nameless dying in early days of her life.Mariam suffered a severe hemmorrhage afterwards,the cause being retained placenta which was very very difficulty to remove.She was brought to the hospital,exahusted and tired,paper white appearance and it was a weekend and a Doctor on Duty was not available in the ward.Luckly enough i was there visiting my wife who has just delivered a baby girl Eileen.After observing that the Doctor on duty was not there then i thought i've to do something to save Mariam's life.I told the nurse,'i'm a Doctor and i think i can help'.At first she didn't agree with me ,she needed to see my ID Card.Unfortunately i didn't have one.I was just Completed internship in Dar Es Salaam and moved to Shinyanga to wait for posting.Unwelcomed by the nurses i just grabed the sterlile gloves and gown from the Nurses hands and rushed to the Bed where Mariam was gasping,after a series of emergency Procedures Mariam became stable,back to life again.....at that point the nurses realized that i was really a medical Doctor.Nearby Mariam there was a Woman Called Fatuma(Not her real name),she knew Mariam of course and they were neighbours to our suprise.She was just delivered a Seventh baby without any Maternal Complications.After talking to her for some time i discovered that she was poor even more than Mariam,then i kept asking myself 'Why people having the same resources,others make good use of them while others are not???.I asked the same question to Fatuma ....to my suprise the practises she explained are those what i currently call Positive Deviant Behaviors.After some time i attended a Positive Deviant/Hearth Training and started offering technical support to one of our Programme area which was implementing the Nutrition Project among Underfives using the PD/Hearth approaches...very successifully.Then i thought the very same idea can pbe used in Maternal health issues ...of course as ANGELS OF CHANGE PROJECT.

Majority of Programmes aiming at Good Materal Health are always Generalised and institutioanl approaches.However programmes aiming at Buiding Capacities of women to become the Angels of changes for their lives by observing and learning from peers who have the very same resources but having Positive deviant Practices are very few if Any.Angels of Change Projects is a Socially,acceptible and centered Projects which uses socially driven practices which are Positive to build capacity of expectant mothers in Materal Health issues.

Man-making education is a rare and non-existing idea, after Swami Vivekananda's intense teachings around the world. Men and women around the world are grabing information and degrees and do their work for their bread alone. Nobody is interested to teach the poor and the ignorant about the real knowledge to empower himself or herself not only for bread-winning, but an extra-miles going education that can protect him or her from all miseries from outside and also from his or her own inside. big sports festivals like Olympics, World Cup Foot Ball, Wimbledon Tennis or World Cup Cricket, the numbers of viewers are immensely viewing.

I happened to view so much of television news and sports channels. By establishing an environment for social good to occur via television & balance thru mobile phones today, our impact will only increase over time.

How much will it cost to launch your idea? (This can be an estimate)

It will cost around $ 1000 per state in India(29 states). Based on the media we advertise this message it will come down or up. Campaigns are only attracted to limited coverage. But door to door campaign only thru TV/Mobile

What makes your idea unique?

The village laborers are involving their most of their time in fighting with each other, communal clashes, sex addiction, drug addiction, used by Political meeting(by giving some amount and one day meals). They are practiced to avail all the benefits on FREE. Hence, they are not ready to do hard work and wanted to earn much profit on a short period. Due to this they are using unwanted chemical manures to spoil the health of humans. The practice of wasting the time by seeing the TV serials, chat with another, etc. will be reduced. They practice to get some self awareness while doing any work on group. Sharing their views on various matters will lead to healthy environment.

What impact have you had?

Every person should allot some space or at least plant in mud pots will give strenth to the family members. They can get the fresh vegetables or greens from their kitchen garden itself. Or they can use their neighbour houses for this purpose. In which, the relationship will be developed. The waste water, vegetable wastes are putting as manures for the small plants. Communual violence, idleness, etc. will be reduced. By this formation of Kitchen garden Group, the real economically backward people will benefitted. Helping, compassion, Our future generation (our children) should know the real culture and helping tendency, compassion on fellow humans and the fruit of the Spirit such as love, joy, peace, patience, kindness, goodness, faithfulness, gentleness and self-control.

Problema

The strength is fresh vegetables. The weakness is regular quantity will not get from kitchen garden(for such option, we can use the fresh veg market). Threat will be coming from big farmers or villagers. Opportunity will be increasing as well as the health of the family members will develop. I am expecting a bright future generation if we use this Kitchen Garden Group.

Actions

The kitchen garden techniq to be adopted as hobby. Physical work may increase health and the concentration on work will give them mental health. This work to be undertaken at least everyday 30 mimutes. In which, the family will get weekly 3 to 4 days or even everyday vegetables.

Results

Self Help Group may be arranged and around 10 to 15 ladies will assemble in a house, where sufficient place is available for planting such kitchen garden veg & greens. The income and expenses may be shared among them. This unity will give them psychological strenth.

What will it take for your project to be successful over the next three years? Please address each year separately, if possible.

The idel time will be reduced and the practice of FREE to be vanished from the minds of the housewives and idle gents. Regular plantation and understanding will be developed among one another without seeing any caste, creed and colour.

What would prevent your project from being a success?

I believe the most of the village labours are practiced idleness, hence they sit and drink tea shops and arrack(tasmark) shops. Hence, the crimes are increasing. They might be the prevented this and this can be tackled by their wife's and family members. Even drunkards are reducing.

Every Goverment extending so many benefits to the farmers, but actually they beneficiaries are big and corporate farmers.
