I'm a bit confused about what the games do. Do they teach kids how to do the calculations required to determine the glucose levels various foods will create in the bloodstream, or do they help the kids to use the phone as a tool for performing those calculations? Regretably, my inability to read Japanese limits what sense I can make of the "play" you present in your video.
I could not put many descriptions about "how to play" due to limitation of word. Please see following description.
INSULOT is designed for kids to acquire clinical sense with understanding clinical uncertainty. INSULOT is designed for learning the relationships among plasma glucose level, food (carbohydrate grams), insulin dosage, and exercise. To provide step-by-step learning, INSULOT has three different game modes: Learning, 10-Games and Exciting.
The learning mode instructs patients on how to play the INSULOT game (former piece of the video). The 10-Game and Exciting modes are almost identical except for the scoring methods. In the 10-Game mode, players play slot machines 10 times and compete for the highest score. In the Exciting mode, players bet units of insulin to the slot machine and play the game until they run out of insulin units.
INSULOT is a specially-designed three-window slot machine. The first window (a) demonstrates pre-prandial plasma glucose levels. The second window (b) shows food that players are supposed to eat. The third window (c) is a blank where players enter insulin dosages to achieve an appropriate postprandial plasma glucose level. Although it is not realistic, the insulin dosage can be adjusted in steps of 0.1 unit that enable players to adjust the dosage in detail in order to enhance the entertainment value.
A player finally starts the slot machine after entering the insulin dosage in all the windows. INSULOT then simulates postprandial plasma glucose levels, taking into consideration clinical uncertainty, and demonstrates various images based on the appropriateness of the postprandial plasma glucose level. Detail algorithms for the calculation are described in the next section.
The simulated postprandial plasma glucose value can be displayed to verify the results. Table 1A shows the images displayed according to the simulated postprandial plasma glucose values. The final score of each play is determined by a combination of three images in the windows. Table 1B shows examples of the combinations of images and their scores. The game is terminated if three ambulances are shown.
Followings are translation of the examples in the learning mode, which you saw in Japanese.

-How much insulin will you need before eating?
-Press up, down, left & right to change insulin dose. Then press Choice.
-Now, let's play Insulot!
-Whoa! Your blood sugar level is too high. You need more insulin to get the right sugar level
-Close, but not enough insulin. Your blood sugar level is a bit higher than what it should be.
-Great job! You've chosen just the right dose of insulin.
- Close, but a bit too much insulin. Your blood sugar level is lower than what it should be.
- Hold on there! You've taken way too much insulin. Your blood sugar level is very low.
- You'll get even better when you play a lot. Are you ready?
