Discussion about entry: Guidance on drug interactions for elders

I am looking at entries and adding comments and questions. I agree that polypharmacy is a very challenging situation and making an impact can prevent a lot of harm. I am unfamiliar with the Belgian health system and wanted to ask - how do physician-nurse-doctor relate in the prescribing of medicines? Is questioning or discussion of new prescriptions the norm in the Belgian system? Are most doctors with electronic health records where interactions are checked against, or does this happen at the level of the pharmacy or not at all?
