Hi Isaac,

I really like your idea because it recognizes that many women either have existing preferences to use TBAs or constraints that limits their options, and you are identifying a solution to try to work with that preference and make it a safer option for women. You identify that it might be difficult to get the government to work toward monitoring and working with TBAs in this way, and that TBAs themselves could either be apathetic or resistant to learning/ participating. What are your ideas address these potential challenges? Are there incentives to create a more active partnership with the government? Are there ways to engage TBAs so that they will desire these new skills and working in this system? Would there be any negative consequences to them working within the system and being monitored that this would create and provide disuade them from participating?

Thank you, Jessica for your comments. Your observations are quite valid. My response to them now follows:
The potential challenge of apathy or resistance to learning/participating would be addressed by approaching the TBAs through their Union. It would involve Advocacy to their Union Leaders and the discussion would adopt moralsuation. They would be made to understand that the training would be in their interest and would eventually lead to the legitimization of their practice by issuing them Certificate/Licence. Therefore, who ever that did not pass through this training may not be able to practice since such practitioner would not be accorded recognition by their union and Government as well as the general public. Another incentive/encouragement would be by providing the trained TBAs with basic tools/equipment and consumables after the training to enhance their work and only the trained TBAs will benefit from this.

On the part of Government, creating an enduring partnership would not pose a serious challenge. As I mentioned in the entry, advocacy visits would be carried out to the relevant authorities, and this would be to reemphasize the obvious because Government is fully aware of the existence of the TBAs as well as the importance of their work. Government also knows that the TBAs have come to stay and that they enjoy substantial patronage, part of it is as a result of gross inefficiency/inadequacy of government own Health System. The government know that the TBAs are essentially bridging the gaps that exist in the Primary Health Care System. Therefore, it would amount to negligence on the part of government to pretend that the TBAs are not performing a strategic job. As a matter of fact, the TBAs are highly recognized hence some Non-Governmental Organizations (NGOs) working on Maternal Health have been giving them one form of training or the other.
