TOTAL HEALTH CARE PROGRAMME: A new approach: Sulabh Total healthcare programme including sanitation, safe drinking water, hygienic practices and various health care programme developed by Sulabh helps reduce morbidity and mortality due to water and sanitation related diseases, especially among urban slums dwellers. Sulabh advocates and practices the system whereby health cannot be viewed separately from the other essential components that have an effect on the over all well-being of an individual and a society.
Context for Disruption:
The lack of appropriate sanitation, safe drinking water and proper hygiene is the leading cause of diseases in developing countries even in the 21st century. Common diseases due to insanitation include various Diarrhoeas, Trachoma, Malaria, Schistosomiasis, Intestinal Helminthes (Ascariasis, Trichuriasis, Hookworm) etc. which constitutes a major portion of the Global Burden of Disease. Diarrhoea alone claims over 1.8 million lives each year, in particular children below the age of 5 years.
Delivery Model
Sulabh toilet complexes have been used to provide counselling, prescription, laboratory testing, immunisation and medicines thus developing a replicable model covering various modes of treatments – curative, preventive and rehabilitative for the low income groups. Sulabh toilet complexes provide the ideal platform for education, awareness and publicity about essential components of total healthcare programme so that the incidence of diseases is reduced. Large number of persons visiting Sulabh toilet complexes is benefited by this model which includes various systems of treatment viz. Homeopathic, Ayurvedic, Unani, Naturpathy and Yoga etc.
The users of toilet complexes and their families receive cost-effective treatment from various common health problems due to the model following the 5 “A” approach:

They are also educated for prevention of HIV / AIDS, use of condom and contraceptive tablet for population control and participation in public health programmes like Pulse Polio Immunisation Programme etc.
Key Operational Partnerships
Sulabh International Institute of Health and Hygiene (The Health Wing of Sulabh International) implements health projects through support of the parent Organisation and also in collaboration with agencies like WHO, UNICEF, USAID, PFI, PSI, Government of India etc.
Financial Model
Most of the programmes and activities are conducted independently by Sulabh without any external support – financial or otherwise. A few projects have been successfully completed with aid and support of UN agencies and the Government of India.

Sulabh International is a self sustaining NGO which generates revenue through nominal fee levied from users of Sulabh toilet complexes. Sulabh does not receive any grants / aid from any organisation in the world.
Effectiveness
The programme is made effective by introducing innovative approaches like Family Medical Folders – where medical record of each family member is maintained, annual check ups are provided and the record is updated to ensure detection of diseases in early stages. The incidence of diseases has reduced significantly in urban slums and rural areas covered under this programme. It has generated a sustained impact on the community to adopt and promote preventive measures to ensure improved health leading to a better quality of life.
Which element of the program proved itself most effective?
We have given priority to training and education of women to ensure better health of the community – promotion of safe drinking water and prevention of HIV/ AIDS, reproductive healthcare. Training of one woman leads to awareness and care to entire family whereas training of a man ensures training of one person only. The trained women are effective change makers within their family and community because of their role in household chores and child care.

Approximately 1000 families covering over 5000 people.
What is the potential demand?
Our potential demand is to cover urban slums, rural set up and school to take up various programme and practices to improve community welfare. We recommend construction of composite community care centre which include Sulabh toilet complex, safe drinking water, health centre and training programme.

What are your two main challenges to finance the growth of your initiative
The total healthcare programme has not penetrated the healthcare delivery mechanism of the government of India. This is primarily because preventative health care (provision of safe drinking water, appropriate sanitation and waste disposal, and hygiene) has not been integrated into the system. Also, at the policy level the existence of separate Departments / Ministries catering to public health and to service provision (water, sanitation, waste disposal etc) results in lack of consistency in the approach and implementation of public health services.
How did you hear about this contest and what is your main incentive to participate?
By e-mail and our motivation to showcase the innovative total health care practices of Sulabh International at a global level and promote implementation and replication of this methodology in other developing countries.

The recognition by developing country governments to integrate hygiene, sanitation and safe drinking water as major planks for the preventive healthcare policy (based on Sulabh total healthcare programme) would go a long way in reducing morbidity and mortality due to water related diseases.
