This is a great idea with a lot of dedication and resources. I especially like the way you have partnered with Clayton State University for the exercise component of your program and Southern Regional Medical Center (SRMC)’s Diabetes Resource Center and Hospital Foundation to reduce the cases of obesity observed in the SRMC Outpatient Nutrition Program. You might think about adding jump rope as a component. This has been done successfully in Indiana, and specifically to fight youth obesity:

Clarian Health has been named among the "Best Hospitals in America" by USNews & World Report for five consecutive years. Clarian is the result of a cooperative effort of three downtown Indianapolis hospitals to combine resources, research, staff and administrative procedures for the greater benefit of our patients and the community at large.
