According to the WHO, the continuity of care during pregnancy, childbirth and breastfeeding is one of the most important factors when reducing maternal, fetal and newborn mortality and morbidity, as it increases the chances for a low-risk pregnancy, a healthy childbirth and higher success in breastfeeding.
We intend to create 3 groups of midwivesm abd each group will be responsible for a number of clients, ensuring the continuity of care in pregnancy, childbirth and breastfeeding. Each pregnant woman (or couple) will have a designated midwife who will be close to the other two who integrate her group, so as to ensure the continuity of care should any impediment arise for the first midwife.
Assistance will be available 24h/day, 7 days/week, during pregnancy, childbirth and breastfeeding.
Among the functions of the responsible midwife/group are the vigilance og the pregnancy (in cases of low-risk pregnancies – high-risk pregnancies are taken to the adequate health services), prenatal education, supervision of birth labor, attention to the parturient and newborn, counseling and supervision during breastfeeding. It includes homecare services whenever necessary.

In Portugal, the current health assistance model for mothers and newborns does not consider the need of continuity in care. That is, the pregnant woman is supervised by one or more health professional (doctors, nurses, midwife nurses, doulas, etc.), then goes to the nearest hospital when in birth labor without know ing any of the professionals who work there, and after the birth of the baby there are other professionals the parturient does not know. The complaints of the pregnant women are evident and clarifying about the need of new projects and ways for care.

Quais foram os impactos obtidos até o momento?
My short experience as an Independent Midwife shows a significant increase in maternal and couple satisfaction regarding assistance, support and counseling received during pregnancy, childbirth and breastfeeding. I have seen that women who have this supervision show better breastfeeding rates than the national average.

The medical community in Portugal does not support the supervision of pregnant women by Midwives, which could present a barrier. However, legislation that protects this situation is currently in process of being altered in favor of the competencies of the Midwives.

For me, being a Midwife is much more than being in a Childbirth Room. In Portugal, the figure of the midwife is almost exclusively associated with the childbirth room, which made me feel the need to change something when I graduated. I imagined this project motivated by my own dissatisfaction about the social vision of my profession, by the national indicators – notably the high rate of cesarean sections and dystocia – and above everything else by the dissatisfaction shown by women regarding the received care during pregnancy, childbirth and post-partum, and I believe it will be successful.
