What makes your idea unique?

It is a successful attempt in the history of social marketing that misoprostol is used for the prevention of excessive bleeding (PPH) during home birth. The success was achieved through the mobilization of local women and use of Sangini Franchising Network. Local women after orientation were termed Community Change Agent who conducted orientations to women groups which led to use of misoprostol during home deliveries.

What impact have you had?

The impacts of the program were observed through the monitoring and evaluations done by IPC and CRS staff in Bardiya district. The impacts were;
• Out of 268 women interviewed, 78% of the users had overall knowledge on Misoprostal, 22% had knowledge on when to use and how to use.
• No casualties were found and only small percentage of women complained of headache, fever and severing and diarrhea.
• Out of 266 deliveries, only 35 women went hospital to deliver, rest of the deliveries were home deliveries and 2 of the women had not reach their delivery date in the time of monitoring period.
• 48% of the buyers of MSC were found to be husbands, 28% were relatives and 24% were pregnant women themselves.
One of the CCA reported a touching incidence about her neighbor dying in cowshed due to excessive bleeding after giving birth to her child. She said that, “if Matri Surakchya Chakki was available at that time my neighbor's life would have been saved.”

Problem

Approximately 250 words left (1200 characters).Maternal mortality rate of the country is 281 per lakh total live birth. Majority of the death during delivery is due to Post Partum haemorrhage (PPH) which accounts for 47% of total MMR (National Maternal Morbidity and Mortality Study 1998).
Approximately 79% of women deliver at home, less than one-fifth of births take place with the assistance of skilled birth attendance (SBA), and women receive assistance from a relative for nearly one out of every two births that probably have not had any formal training on deliveries and birth preparedness. Seven percent of births take place without any type of assistance at all. The situation is harshest for less educated and poor rural women, compounded by the insurgency and internal displacement over the past 10 years. Access to health facilities and trained providers remains a challenge, and while the GON is focusing efforts to increase the percentage of deliveries in health facilities, the need remains for interventions that seek to ensure use of Misoprostol for the prevention of PPH in household deliveries.

Actions

Implementation in Bardiya District(Pilot Initiative)
For the implementation of the program, MSC were made available in the 46 sangini outlets of Bardiya districts and orientations were held for the sangini outlets and CCAs accordingly.
Orientation on Misoprostol to Sangini Outlets:
All the 46 Sangini Outlets of Bardiya were given one day orientation program on Misoprostol. The orientation program was held in phase wise to effectively communicate the responsibilities of MSC seller.
They were asked to record the basic information of the purchaser and also the user of MSC. They were provided with logbook to record by Nepal CRS. They were also required to proper counsel the purchaser and they were strictly informed not to sell MSC if the pregnant women has still not cross seven months of her pregnancy period.

Further, CRS in coordination with NFCC developed the reporting and monitoring tools for CCA orientation program. A joint team observed the orientation session conducted by CCA and provided necessary feedback.
A total of 290 sessions of community meetings were conducted by the CCAs to women groups of Bardiya district.

Results

In Bardiya district, the use of misoprostol and overall knowledge were found to be high and also some of the female went for institutional deliveries and no casualties have found till this date which major achievement for this program

What will it take for your project to be successful over the next three years? Please address each year separately, if possible.

Over three years, this program will cover rural areas which will increase use of misoprostol in home deliveries. Similarly, in terai areas where health services are available, we will promote institutional delivery which will ultimately increase the number of instiutional deliveries.

What was the defining moment that you led to this innovation?

Nepal CRS's objective to contribute in the field of maternal and child health let to come across with this idea of use of MSC in home births as it would take long time for rural and remote people to go for institutional delivery.

Mr. Krishna Bahadur Rayamajhi who has been working in the field of social marketing through Nepal CRS Company was determined to create a sucess in the field of maternal health by encouraging use of MSC in home births which is high in the context of Nepal. Pilot Project was intiated in Bardiya district as Bardiya's observation and distribution network of CRS was strong.
And, success was seen as use of misoprostol was used and no casualties were observed.
