The Need: What problem are you trying to solve?
Three major causes of high rates of maternal and infant mortality in rural areas of Pakistan: lack of maternal & infant health literacy, access to care, lack of skilled, trained and well-equipped health workers at rural levels.
1.Health literacy: in rural areas there is low maternal health literacy, birth preparedness or complication readiness. Many families and LHV’s/LHW’s also do not recognize high risk sign and symptoms of pregnancies for timely care or referral. Also, due to poor knowledge importance of institutional deliveries limited number of pregnant females delivers in the hospitals.
2.Poor access to maternal & infant health-care services: There is shortage of skilled, motivated and well-equipped health-care workers at village level.

The Solution: What is your solution? Be specific!
To reduce maternal and infant mortality, this project will create a network of village based LHV’s/LHW’s to adopt a holistic, decentralized, community based approach. This network of health worker will create demand for maternal and infant care by creating awareness and meeting the demand by increased access to the care through an effective referral system. This project aims to prove that service delivery through village based network of female health workers under the supervision of gynaecologist at secondary & tertiary care hospitals will be a cost-effective, scalable and sustainable approach to maternal and infant mortality reduction.

Objective 1: Increased Awareness
Strategy 1.1: By developing network of village based lady health workers and lady health visitors who will sensitize community about the need and importance of timely maternal and infant health care.
• Identification of LHW’s/LHV’s in the intervention zone
• Capacity building of LHW’s/LHV’s for standard safe motherhood and child practices inclusive of family planning
• Community sensitization

Objective 2: Increased access for maternal health-care
Strategy 1.2.1: Provision of antenatal check-up and post natal check-up for the registered pregnant females at the basic health units and through home visits respectively.
Strategy 1.2.2: Motivating & educating pregnant females for institutional/attended deliveries and timely treatment for high risk pregnancies.
Strategy 1.2.3: Creating back-end linkages with the local government hospitals and institutes for timely referral of delivery
Strategy: Creating a supporting network of Tele-consultation, through LHW’s/LHV’s with the government hospital in case of emergency
Objective 3: Increase Referrals
• By capacity building of the lady health workers to rule out complicated cases
• Developing back-end linkages with the secondary and tertiary care hospital through tele-consultation
• Provision of transportation

currently no one working specifically on the proposed idea of making a network and connecting LHWs/LHVs with the secondary and tertiary care health facilities. currently MNCH department operating LHWS/LHVS program of the country,this project will be entirely in collaboration with the health department and MNCH department. There are less chances these departments will pose any problem to the growth of project. gynaecologists motivation is of sole importance to take part in the program actively

during attending global maternal health at terry town, New York, I got chance to discus my idea with the maternal health. they were really appreciated the idea as a potential solution for addressing maternal and infant issues in Pakistan
Please describe the goal of your initiative; outline what you are trying to achieve
1. Empower communities through maternal and infant health: Village health workers will work with pregnant females, their caretakers and family, women self-help groups (SHG), and local leaders to build their capacity to identify and provide necessary care during pregnancy, at delivery and post birth.
2. Bridge the gap to access to maternal & infant health care services in rural areas: Project will increase community access to maternal & infant care and government health services (both preventive and medical) at the local level in rural areas by increasing the demand for services by empowering local communities with knowledge about maternal care and by working with the government at multiple levels to make sure that the demand for services generated is met
