The essence of this idea is to train a nurse, birth attendant, or health care professional within the community. By doing so, I think women will be more vigilant when rupture occurs to seek health advise and be more likely to trust the birth attendant or nurse as she is also part of the community. Rather than a project that requires a lot of external personnel who may leave at any point in time. I also suggest checking on those that are trained to ensure that they keep up recognition of symptoms.

Reason:
My idea is recognition of preterm rupture in the areas where tertiary care is not available and antibiotics is not administered, if one can train nurses or traditional birth attendants to perform a proper vaginal exam at preterm rupture to see if there is amniotic fluid present than they can survey the likelihood of infection. Based on gestation and maternal age and if a safe birth can be performed then, or if fluid is recognized and the women is correctly identified as high risk adequate arrangements can be made to either administer antibiotics if they are available or transfer to a more suitable facility with enough time to treat the infection before it causes irreversible damage or death.
