The Need: What problem are you trying to solve?
There are no facilities and provisions of knowledge sharing other than limited training for the service providers. The local program managers are not commonly exposed to knowledge and information other than own boundary. On the other hand, the local NGOs, the practitioners are willing to explore further knowledge and are keen to share its own lessons learned for improving quality of care, increasing capacity, increasing access to health care and reducing maternal and child health morbidity and mortality. It is agreed that exchange of knowledge and effective sharing has role to improve capacity of the practitioners since it is a self learning tool.

over the past one year ACCESS Health International, Bangladesh identified and documented number of community based health care interventions. Limited access to online facilities, poor connectivity and lack of coordinated efforts causes poor knowledge sharing to learn more on best practices and lessons learned from different low cost high impact interventions available around the globe. Accessibility to online information is not common for the practitioners. There are no facilities and provisions of knowledge sharing other than limited training for the service providers. What is your projected impact over the next five years?
The proposed project takes care of sustainability through applying best practices both in demand and supply side. Since the total process aims to involve home grown solutions without much external involvement, the gathered knowledge and learning would be retained longer. A pool of local resources play a vital role in improving service delivery systems and can further contribute towards quality of care and capacity enhancement from the supply side. The improved, more responsive and better equipped service providers and practitioners can expect higher financial and technical supports leading to sustainability of the program without external donor involvement.

Hi Nadira, Glad to see you are thinking innovatively about health care in Bangladesh! I'm interested to learn more. From what I can tell, ACCESS Health brings health practitioners together in regional workshops to share best practices. How will these be organized? Will they be focused on certain topics, such as best practices in maternal health or elderly care? What will be the practitioners' incentives to go and how will they afford the trip? What will they do with the information after the workshop? Lastly, how will this workshop model be financially sustainable, outside of donations?

Thank you very much for your comments on my entry. I hope the following para will answer your questions:
---• Resource mapping to identify organizational and individual practitioners using CHMI database and other local sources; capacity will be assessed in resource mapping from reported result using CHMI database and probing with formatted interaction developmed by ACCESS Health.
---• Based on the current case reports, and profiled programs from CHMI the best practices will be compiled for increasing access to quality and affordability in Quality of Care, micro insurance, mobile technology, use of ICT and Public private partnership for providing primary health care and maternal and child health at community level. Practitioners can be asked to present their work on a pre-designed format focusing on achievement and areas for improvement at community level primary and MCH care. The audience will comprise of researchers, primary health care and MCH practitioners and technology partners. (Being limited to the three to four for better discussions). Ideally some gaps in the healthcare delivery expected to be identified and how these organizations have bridged it. A SWOT analysis would be done of the proposed solution by the practitioner themselves.
--• Following the presentation a discussion on these best practices will be initiated by ACCESS (in partnership with relevant organization) in order to understand which are most feasible, can be scaled, most effective and more sustainable etc. Once this analysis is done, an assessment of the most needed and feasible option will be to be explored for adoption or replication or scale -up.
--• The workshop can conclude on how organizations can help each other in replicating of the strategy. It can also start a dialogue on how to engage a continuous discussion post this e.g. creation of a knowledge forum chaired by local organizations could be an option. This forum would meet periodically to share information. ACCESS can initiate and facilitate this and parallel create systems to make it an ongoing activity.
--ACCESS’s role would be to facilitate the knowledge transfer especially to improve processes and training of people to improve access to healthcare.
The ACCESS Health has planned to mobilize resource for this initiarive to finance the admistrative cost, the trip and workshop logistics with other justified cost. Learning and exposure to new knowledge and skills are the most important incentive for the practitioners. Further, the practitioners will be profiled in ACCESS and CHMI websites. An impact assessment will be planned after workshop to see the changes among the the practitioners. Since, there is a demand for continued eductaion for the practitioners , it is assumed that practitioners will be able to manage its own funds or allocate resources for its owm improvment and capacity enhancement.
