¿Qué hace única tu innovación?

In Mexico, a lack of an efficient system of primary care leads to drastic consequences for the health of low-income families. When people do not have regular access to a physician, they tend to wait until the problem becomes an emergency or a chronic disease, leading to results that may be catastrophic. For instance, 80% of kidney transplants performed in Mexico could have been avoided had the patient sought proper care earlier. Moreover, the number one cause of death in Mexican women is cervical cancer, even though it can be easily detected and cured in its early stages. Additionally, waiting until health problems reach serious levels has significant financial effects for Mexicans. Primedic seeks to counter the culture of reactive medicine in Mexico and promote affordable primary care, thereby improving health outcomes.
A Primedic membership costs a fee of MX$1,199 (USD $92) per person per year. In return, subscribers receive free, unlimited consultations at Primedic’s clinics. Also, routine diagnostic procedures (mammograms, x-rays, etc.) are available at dramatically reduced prices. Morevoer, patients can see other medical specialists through Primedic’s referral network, where they pay half the price of a regular co-payment. Even though public health care exists in Mexico, it often includes very long waits (for instance, 2 weeks for a CT scan) and frequently lacks medicines. Primedic offers the quality of private clinics at a cost that is affordable to low-income Mexican familites.

¿Qué impacto has tenido?

Students from the Harvard Business School performed a case study on Primedic and were able to talk with several Primedic subscribers. One subscriber, a 30-year old female and small business owner, had purchased membership for her son:
My son has epilepsy, diagnosed 12 years ago. No one ever explained to me how to care for [my son] and update his medicines; none of the doctors ever followed up to check on him. The doctor’s office visits were very expensive, and we haven’t seen a doctor in 5 years. But in the last several months he’s been very sick again. I was very excited to hear about Primedic; it has solved a lot of problems for us. So far, the doctors have made the office visits very convenient, based on my child’s school schedule. Everyone at Primedic has been very, very careful in treating him, double-checking everything, and they treated us with respect every time we visit.
This interview demonstrates Primedic’s main impact: providing affordable and accesible healthcare to Mexican families. Another important impact is Primedic’s role in promoting preventive rather than corrective medicine, which leads to more succesful health outcomes. In the past 12 months, Primedic membership has grown over 50% from 2,400 clients to 3,800 clients.

Problema

The problem that is being tackled by Primedic is Mexicans’ lack of access to high-quality primary care services. It is estimated that more than 40 million Mexicans have insufficient access to healthcare services. The healthcare services offered by the government are so limited and bureaucratic that, even when they do offer preventive healthcare services, most of them do not reach a large part of the population. Sixty percent of health costs in Mexico are paid out-of-pocket because of Mexicans' preference for the much faster and higher quality of the private system. However, private healthcare can be unaffordable for low-income families, leaving them with few options. Primedic strives to fill the healthcare gap through its affordable, high-quality care. By providing primary care to low-income Mexican families, the occurrence of serious health problems can greatly be diminished.

Acciones

Recently, Primedic formed a partnership with Ignia, a Private Equity Fund, as well as Tango Group, a co-investor. This brought growth capital to Primedic that allowed the company to expand to a total of 6 clinics, which are located in the northern Mexican city of Monterrey. Additionally, Primedic has brought in much expertise into the company in the areas of finance, marketing, and administration in order to ensure its continued growth. It was recently featured as a Harvard Business Case study. Also, Primedic is in the midst of negotiations with several large Mexican corporations that will allow its growth curve to accelerate more rapidly. Primedic focuses on innovating new ways to reach the bottom of the pyramid through strategic partnerships, microfinance, and marketing. Ultimately, Primedic aspires to have a presence nationally throughout Mexico and fulfill the need for accessible primary care.

Resultados

With the financial support of several investors, Primedic is continuously searching to find the formula for success with regards to sales of new memberships. The first 2 years of partnership with investors were challenging, yet restructuring of the company has allowed it to expand its expertise and improve results. For instance, over the past 2 months, sales of memberships have quadrupled. As far as the corporate channel, Primedic is currently launching a pilot project with a global company in Monterrey. This partnership will represent a large leap in memberships, with an anticipated 6,000 new clients. Additionally, Primedic has partnered with the biggest microfinance institution in Latin America, Banco Compartamos. This partnership will begin in September and will make a Primedic membership even more affordable through easy access to micro-loans. These strategic partnerships will help to broaden Primedic’s reach to low-income families.

For the first year, execution with planned partnerships is crucial in order to compensate for losses from the previous management team and continue to expand. For instance, a new contract with an international company in Monterrey will lead to a significant growth of 6,000 new clients. Additionally, another contract with a microfinance institution should lead to increased sales due to greater affordability of a Primedic membership.
In the second year, the focus will be on maintening the average monthly sales per sales representative in the direct channel and to obtain 2 other large corporations in the corporative channel. We will also start thinking about opening new clinics in other cities.
In the third year, we plan to consolidate the company and will strive to have a consistent annual growth of 20%.

Initially, there were poor results and many problems with the former management team, which affected the patience and tolerance of the shareholders and brought additional tension to the equation. However, the current management team is working towards improving the company in all areas: marketing, finance, human resources, and innovation. Hence, the management team plays a crucial role in company success. Additionally, lack of financial support would impede the growth of Primedic. Thus, support from investors is critical in allowing Primedic to expand its services to new clinics and reach more patients, especially those from low-income families.

In the healthcare industry, everything centers on trust and certainty for the patient regarding the provided service. This sense of trust is often spread by word-of-mouth, which is the strongest channel of communication for Primedic services. Hence, forming networks and alliances with trusted organizations helps to give Primedic more value and be more trusted by the patient. It allows Primedic services to reach clients of the other organizations with which we form alliances. Thus, partnerships help to attract new clients, increase credibility, and often provide financial support, as well.

First, in order to grow as a company, it is necessary to execute and fulfill the expectations of our clients. Second, we must gain their trust and be able to convince them that their wellbeing is our primary concern.
Finally, it is important that we start to change the culture of healthcare in Mexico from a correctative viewpoint to a more preventive attitude. Our Mission Statement says that this change has to come from inside of Primedic to the community to which we belong. It states, “We aim to improve the quality of life of the community and create a culture of prevention in health to favor the human development of the person”.

¿Cuál fue el momento decisivo que te llevó a desarrollar esta innovación? Cuéntanos tu historia

My background as a surgeon working in various public and private practices awakened in me the quest for less expensive ways to achieve excellent health results. As a doctor who is passionate about people’s wellbeing, I was and still am very upset by the existence of physicians with motives other than the health of people. These experiences have taught me that a good student or practitioner does not necessarily turn into a good physician as the system generates and fosters dreadful incentives. Also, I noticed the great lack of preventive care in Mexico. I witnessed many women suffering from cervical cancer who should have received medical assistance long before the disease became malignant. That is why I have decided to look for a change in the established system. Thus, with the collaboration of five other renowned physicians, Primedic, former Transparencia Médica, was born in 2002. The initial investment cost 600,000 pesos ($46,000 USD) and initially there were 39 patients. Today, there are 6 clinics in Monterrey, Mexico, and around 3,300 patients. Fortunately, Primedic is continuing to grow and I hope to take it to a national level in Mexico to help provide primary care to low-income families.

I was born, raised, and educated in Monterrey, Mexico. After I finished medical school, I pursued a specialization in surgery. After graduating as a certified surgeon, I worked in several areas of the healthcare system, which gave me a wide perspective on its various deficiencies. When working in the Public Hospital National Medical Center IMSS Siglo 21, in Mexico City, I realized that several patients had not received proper treatment, since they should not have reached the third-level of seriousness with their diseases. I also spent several years as a professor of surgery at the National Medical Center and in private hospitals (ABC and Ángeles in the D.F.), as well as in the University of Minnesota in the United States for a two-year program, before returning to Monterrey. When I returned from the United States, I worked in my own private practice office for four years and at different hospitals, both private and public. Through these experiences, I was able to observe the care received by patients as well as the financial expenses incurred in the health business. Finally, I have worked as a consultant for insurance companies, where I learned about how physicians often prescribe unnecessary medicines and tests.
