Vancouver' Inner City (over 50 City Blocks) records some of the lowest income postal codes in Canada. Home to over 3800 children it has some of the highest vulnerabilty in the province with 66% considered not school ready. They face crime, social disorder, unsafe living conditions,have ongoing food insecurity, unstable housing;are excluded from needed Early learning/care and enriching childhood opportunities.

The Traditional service approaches -Universial programs are executed uniformly assuming infrastructure/ capacity that does not exist where vulnerability is defining both the children and the neighborhood. The current system of silos fails to provide the required: mix of program/services;intensity;or differing approach to meet specialized needs of neighbourhood & children.

Complex problems require comprehensive solutions, as Community organizations, residents, institutional and local partners, we committed to share resources and capacity and build a shared "place based plan",with measurable outcomes related to a shared Vision: "To ensure that Vancouver’s Inner-City Neighborhoods are places where children and their families are exposed to a wide variety of early childhood and family enrichment opportunities; where children and their families have access to needed resources that promote optimal health, social justice, safety and full inclusion; where school readiness and success is the norm." The Strategy is called the Inner City Childhood Response the Outcomes are: 1. Improved Child Health;2.Enhanced Child Development;3. Improved Family Functioning; 4.Improved Systems of Care;5.Social Justice* 6. Economic Well-being. Each of these areas has specific activities/actions which all partners contribute resources/effort to achieve outcomes.

The Model: Walk us through a specific example of how your solution makes a difference; include the primary activities involved in your solution.

In one example, our clinical data documented that for a particular cohort of children after only 4 months of engagement in activities such as those described in the place based strategy (pre-school,early learning programs, mom and tots, gross motor programs, developmental screening and planning) there were measurable positive differences in the children’s development using developmental milestone baselines. However their parents also reported reduction of personal stress; greater understanding of developmental stages of their child, knew how to modify instruction and expectation to age approperiate levels. They also reported they had increased quality time spent with their child. Several parents formed a Parent to Parent peer mentor/support group (P2P) based on the assumption -"all parents want to be the best parent they can be, and that parents need help in real life situations". In this place based strategy it is the planned, simultaneous, yet connected initiatives using shared resources, and provided by various partners such as parenting programs, housing advocacy & support, food security programs, responsive medical attention to health conditions of the family, and peer support, that secure the best opportunity for positive outcomes. It is a multipronged approach that enacts a commitment to capacity building among children, youth and adults. As a consequence not only will programmes be developed and children receive needed supports, but this approach invests in families and in the neighbourhood thereby enriching the fabric of the community itself.

President Obamas Promised Neighbourhoods, Harlem Kids Zone, Austrailia place based..are all addressing similar issues using a place approach-we consider them allies and are open to all who wish to change in the outcomes for our children and youth as champions and partners. This initiative welcomes everyone to the table-(medical, educational parents citizens,faith groups, communtiy organizations, housing, legal...) all are considered to have capacity to add to the solution. There is a regular community-partner table it meets every week on Thursday-it involves parents, youth, service providers, community organizations, legal, health, primary care and medical practitioners ( this Medical link called RICHER is another innovation within this place-based Response innovation).

A resource rich pre-natal program was operated in the community, but results three years later raised the question- What impact does a great pre-natal program have if the baby is in a family living in housing infested with mold, rats and bed bugs, if that same family does not have access to ongoing primary health care, does not have sufficient food, are often victims of crime due to violence and criminal activity in their neighbourhood, have no funds or access to pre-school or enrichment programs and their child is not school ready. Children cannot succeed if they don't get a fighting chance-In a community like the inner city where over 80% of the families are low income it also means the programs cannot afford to operate and often do not exist. These are complex problems that require far more than a single program or the traditional silo approach of disconnected decisions, funding and responses. Universal policies fail when provided uniformly & not re-aligned to context of place.

Using the Harlem Kid Zone concept based on 100 city blocks "Place" which see a pipeline where a child enters starting prior to birth through all levels eg. early learning, elementry, middle, high school graduation to college supported with the corresponding family social and health programs and community building activities that are linked and coordinated to ensuring every child makes it through the pipeline --- reaching and realizing their potential and with optimal health, skills and abilities to make fulfilling life choices.
We have added two other components to supporting our inner city pipeline for children/youth-Social Justice and Economic Well-being.

Our focus is the inner city-it provides the place based frame in which partners come together to do the work.

Qual foi o impacto da sua solução até hoje?

Groups working together in a very new way-real collaboration not around a Program but around people and place-e.g. open table community meetings weekly for problem solving and needs identifications. A comprehensive plan with outcome,team leaders and a vision. Sharing of capacity, resources,funding approaches to achieve goals of the plan-eg. Community centers give clinic space; connected partner responses-e.g. parenting courses, same time chldren are in early learning or health care program. Highlites: 2011 caseload-977 vulnerable children/youth primary and specialist care-12 Clinics/week, 9 sites; 60+ new pre-school spaces; School Board Strong Start= 42 families. Food security= meals/snacks to 400+ children;110 family food bags/ month. Shared training, peer mentorship programs for parents; connection to employment service. Transition planning to kindergarten. Medical- Legal- Community partnership-e.g. to address adverse conditions creating medical issues for children.

Less silos, more application of comprehensive solutions, where all contribute their strength to create successful outcomes-some recognition and re-aligning of policy to reflect the reality of vulnerable people and the impact of that reality within a given community- or in context of "Place". Healthier Children/ Youth graduating from High school with options/opportuntities for post secondary- able to make full-filling life choices. A community which is more socially connected, aware and supportive of each other. A community where a higher majority of people are now participating as citizens not clients and patients, where children are valued, supported, nurtured- The adoption of the concept and principles that apply to the adage "it takes a village to raise a child."

The ever changing targets and focus particularly of government funding and resource allocation e.g (RFP Process) that dismantles vital community infra-structure in a competitive process without understanding what existed and the impact of these decisions on vulnerable neighbourhoods/communities. Universal policies/programs that are delivered uniformly and do not recognize that not all communities or people have the same starting capacity-so equal does not mean equity-we need universal access, universal standards, and equity of outcomes....current approaches fail some of the highest risk populations and neighbourhoods.

The partnerships involve community citizens, new immigrants, aborignal people, business, legal ( law society, legal aid), UBC School of Nursing, medical( children's, Womens. UBC, Vancouver Coastal), Community Centers, Community agencies, funders-UW, Vancouver Foundation, City of Vancouver and Parks Board. Aboriginal Organizations( ALIVE, Native Court Workers, 1st Nations Employment). local BIA's. The partnership is open to any one/group that wants to work to achieve the shared vision through a comprehenisve approach with the community.

Are you currently targeting other specific populations, locations, or markets for your solution? If so, where and why?

UBC School of Nursing (RICHER) and inner city community is partnering in a proposal to compare experiences,exchange information and learnings between 2-4 different communities using a place based approach as it relates to population health outcomes. There is also efforts to research, share place based experiences in a learning partnership with Victoria groups United Way and all partner groups from the Inner City Response Initiative. The Representative for Children and Youth office is interested in the outcomes of this work. Started work on Medical Legal and community partnership

Que tipo de ambiente operacional e fatores da organização interna ajudarão o projeto a ser bem sucedido?

Desperation -we all know we can't keep doing the same things over and over and expect different results-people know there needs to be a different approach and the vulnerability and serious situation in the inner city are catalysts for people coming to the Table with a readiness to go outside the box. The economic and funding landscape is also one that forces a different model as the realization that together we have more influence with a common vision and a plan.

Champions are needed at policy/resource level to re-assess and in some cases realign policy or provide a different basket of services in communities where there is a high concentration of vulnerability and a mismatch between needs/outcomes that is related to resourcing or incompatable policy application. Resources to support meaninful active representative resident involvement.
