The community mental health models are not is existence in most of us our urban situations. We are among very few organizations that have succeeded in this area. Engaging community in the process of rehabilitation has been the central point of our work this has created impact and resulted in change in the lives of people. We have experienced the reintegration and persons becoming part of family & community fully which was an unique experience. The programme has demystified the typical model of medical approach.

The community mental health project has made severe impacts on the life of a person, family members and in community. Firstly, the most satisfying factor is to see the changed behavior and stabilized health among people with mental illness. This has been achieved through meditation, counseling and various social integration practices. Many people with mental illness after our intervention has totally changed and they started responding positively and meaningfully in their lives. This has witnessed around 30% of them have gone back to their earlier profession within 6-8 months of our intervention and another 30% of them have stabilized and taken up all their own individual and family responsibilities, which has made huge difference into their lives and greatly prevented the further vulnerability and exploitation.

The families had major barriers within the neighbor and community life by managing the various difficult situations that are created by persons with mental illness unconsciously. The support of our project through counseling family dialogues has created knowledge, hope and understanding among family members to take proactive responsibilities in addressing the problems which would help mental illness person to again re-integrating in their own families. As the mental illness persons started responding positively, the family situations have drastically changed very positively. This has in many cases ensuring safe environment, positive responses and taking away all their social pressures, which lead them to be more comfortable and confident to live in their community.

The community response has started changing positively as they seen changes in the behavior and their attitude among people with mental illness. The neighbors and community members started providing shelter, food, jobs and various other supports which enabled persons with mental illness to be truly integrated in the community.

Problema

Our experience by working with mental illness has created great amount of learning experience, as well as created various challenges which enable us to understand the primary problems of this work. Following are the key problems that have been experienced by us.

1. Firstly, accepting of our social workers in the families of mental illness due to various reasons was a key problem; we faced in the initial period of this project. Families always felt that, we could not be able to play any vital role in changing the lives of affected people.

2. Meditation was the biggest challenge the project experienced in the initial period. In the year 2003, the NIMHANS was only available major source for the treatment. This has to create major barrier in ensuring quick medical support. Families used to take mental illness person to the hospital, they have to spend almost a day, their labour & time.

3. The community had negative attitude towards persons with mental witness mostly young people is to treat very badly or behave very violently towards people with severe mental illness. In many occasions, persons were beaten up, thrown stones and treated abruptly.

4. Developing hopes and creating family support at the initial stage was the major problem. Since most of the families had almost lost any faith and possibilities of treating and re-habilitating the person affected. Hence gaining their confidence and co-operation was an issue.

5. Getting support from hospitals ensuring Government support system and engaging community systems to respond to the needs of mental illness was challenging.

Acciones

APD has developed very clear strategic directions to make through success of this project. Following are the key steps that help us to make it successful.

1. Primarily APD along with the support of Basic Needs - India has evolved a clear action plan on the overall concept of community mental health requires structures, staff patterns and working strategies.

2. The project has appointed committed trained workers and knowledgeable coordinator to implement the project in urban slum, as well as in rural communities.

3. The training change has been evolved to ensure the professional understanding and skills that would help our staff to deal with all types of mental illness.

4. Community mobilization and community environment was other key step which enables us to create greater among of understanding, awareness and participation among community members to respond to the needs of people with mental illness.

5. Care givers training programs and care givers consultations was another step, which engaged families to come together to share their concerns, difficulties and best possible way forward to respond to the needs of mental illness.

6. Counseling support from professional counselors both for an individual person with mental illness and the family members has enabled to listen and allow families to come out with possible answers to meet the required support.

7. Networking with Government hospitals, professional organization and with the Government departments has been the other steps which enabled us to get better medical support, Government assistants are working together for policy changes.

8. Constant reviews along with family members, community leaders, our staff and professionals have enabled to evolve required programme and strategies to move from one step to another step.

Resultados:

Over the years, this project has made tremendous amount of changes in the community. Following are the key results.

1. Hopes and aspirations among persons with mental illness and family members has been the first success towards taking this project forward.

2. Medical support has enabled stabilization; this has resulted in changing behaviors and attitudes among mental illness persons.

3. The stabilization process has further enabled people to engage back into their family life both at family life both at family level and many of them re-entered into their professional life.

4. The family support and the medical responds have been strengthened and today there are 5 hospitals in Bangalore, who are providing medical support.

5. Employable opportunity has enhanced among people with mental illness. This resulted in economic independence and creating self respect among the persons with mental illness.

6. Government has taken-up steps towards ensuing monthly allowance, ration cards and medical cost support, which has been the greater success in ensuring social protection.

7. The role models that are now available are the key factor for future drive. People who recovered mental illness and care givers who are the lay councellers and inspirations within the community.

8. The alliance among NGO and the Association of mental illness and care givers has been very successful in strengthening both support system and enabling empowerment process.

Following are the detailed plans for the success for the next 3 years:

In 2009: Creating more awareness through parents of person with mental illness (PWMI), community people, NGOs and Government Authorities. They have to consider the mental illness, is a serious issue for a larger public. For this awareness, we are using Audio, Video, Create Litreatures, electronic media and press media, which will create awareness and sensitize in larger public.

In 2010: Building parents and strengthening the federation. It is ideal way to sustain this programme by providing training, counseling the skill management training etc., To achieve the awareness on mental health act and to run the programme along with pro-active NGOs, we have to contact Taluk level association, care giver association and merge with an existing NGO.

In 2011: Increase the community participation, working with Government and announce policy making. It is on going process, right from the beginning, we shall be working with Government Departments like General hospital, District hospital, State level department and especially Deputy Director of Mental Health Programme of Karnataka. By conducting Adhalat face to face programme, applying RTI and consultation.

Basically it depends upon the will of the politicians, Government officials, and non co-operation of the parents. We are working out on new strategies. It will take a time to come out of the problems facing with all these, but the project will be successful.

APD believes in working with partnership of various stake holders, primarily our stake holders are people with disabilities, Bangalore members, community leaders, NGOs, Industry Corporate and local Government. We have been working with all these groups mainly with an intention of achieving their participation and collaboration to meet the ultimate goal of the project.

In the area of community mental health, APD has a long term partnership with three NGOs, namely, APSA, Paraspara & Basic Needs India. The most of the mental health programs have been jointly organized with the support of these NGOs. Similarly, along with the partnership, we have been able to organize care takers training programs larger community awareness programs and developing association for people with mental illness. Today, Bangalore city has a strong lobby group with an effort of co-ordinated support from partnership.

Industries and Corporate have been always our main key partners in terms of promoting livelihood opportunities and engaging them in their corporate social responsibilities. Particularly in the area of community mental health, APD has developed partnership with one local industry, which has been supporting 10 people with mental illness to engage in vocational training activities through supply of raw materials. Similarly, we have been working with other Corporate towards ensuring employed opportunities and also creating awareness among the employees on various needs of people of mental illness.

Government has been the primary responsible body towards meeting the needs of the people with mental illness. The project has been actively engaging in with the Department of the Directorate of Health Disability Welfare, Women and Child Welfare Department and City Corporate Offices. Their partnership we are promoting social security benefits, support for medical treatment and creating opportunity integration and support awareness creation in the larger society.

APD’s experience has largely gained through its response to the emerging needs in disabilities sector. Being in the field of disability work for last 50 years, the organization has gained enormous amount of experience and also realized the challenges that are existing in the sector. Through out the journey of the APD, it is always believes in the philosophy of installing, self confidence and ensuring self reliance of persons with disability. Hence, for the organization disable people and their needs are the driving force to meet all these innovation. Though APD has started to respond to all needs of physical disabilities in its beginning years, very soon oganisation realized the needs for responding to the other disabilities. Over the period, APD has seen the organic growth which has evolved over the period. Today it has reached 15000 people with disabilities across four districts of Karnataka directly. Similarly, it supports over 30 organisations across South India to enable their capacity to respond to the needs of the people with disabilities.

APD has recognized from people with disabilities as a centre for providing re-habilitation, employment opportunities and enabling them to become integral part of society. The NGO world has recognized APD has a resource organization and technical organization. The Government has recognized APD has one of the expert disability organization to train various Government officials on disability matters and APD has been invited to be part of all major strategic decisions making process of Government in the State of Karnataka.

These are the key areas that lead us for continual improvement and innovation in our work in the area of mental health work and over all disability work. Within the organization, there is a driving force from the Board due to their high involvement and decision making abilities there has been all the time high level motivation in the organisation. APD Board consist over 40% of people disabilities and staff consist over 35% people with disabilities. Hence, in all its work naturally people with disabilities involved in the whole process of designing, implementing, reviewing and monitoring.

As an organization, APD has a philosophy to be open and respond to the emerging needs in the sector. As a result in many occasions, APD had an opportunity to experiment new learning’s and new Programmes. All these factors have been leading reasons for our innovation.

The key people behind this innovation are:
Firstly, the person with mental illness and their suffering needs. Consistently they have experienced the real struggle that has been faced by the family members to which integrate their own affected members. This has been a challenging factor for APD to think innovatively to respond to the existing issues.

Secondly, Basic Needs India which took up a lead in the country to challenge a medical model and to evolve social integration of mental illness, it has played a crucial role in influencing APD by supporting and building the required knowledge, involvement in this work.

Thirdly, the positive results that our staff has experienced while working with the persons and their families and the support we have received from the local communities and from involvement has bee other factor behind this innovation.

Fourthly, APD’s Board, its larger management team of all other programme, peoples encouragement, involvement and their on going support by creating opportunities for people with mental illness to take part in their on going programmes
