About Project

Problem: What problem is this project trying to address?

Today healthcare improvement, community development and patient legislation in many countries put a clear focus on citizen participation, empowerment and choice. At the same time elderly people often suffer from lonlieness, isolation and lack of powerment and control in their life situation. This is a problem which may increase in an aging population. To improve the conditions for elderly in this situation new arenas for meeting,learning and collabortion are made. Passion for life is based on the individual´s own motivation and contexts. The programme Passion for life tries to solve especially the problen with lonlieness among elderly.

Important factors for the implementing of Passion for life are Joyfulness-Personal motivation is released in an environment where there was joy. The personal interviews in the pilote project showed the importance of learning with peers. This created much inspiration and deeper learning. Usefulness-the methods were simple and user-friendly! The group´s own experience is used in a wide perspective. The seniors themselves are the experts. A very imortant factor is the Life Wheel, the so called Deming tool for improvement work. By working with such a tool of improvement the seniors learnt a structured way of solving problems. Empowerment-to feel empowered must be experienced and made visible to be real- .Ownership of the situation is necessary for a change Reflection-quality improvement tools and methods are helpful when moving from words to action.

Participans in Passion for life meet in "Life-Cafés" led by one or two circle leaders. The circles consist of 5-10 members. Each Life-Café focuses on a selected topic related to WHO research on what effects health and quality of life in elderly;empowerment, safety at home, food and drink, social network and physical exercise. Within each area the participants identified gaps between the current situation and how the best possible situation could be. All made measurements and tested changes at home to reduce the gaps they had identified. The PDSA-cycle was the tool used. A special Café letter was sent to the participants after each café as a summary. At the last Life-Café a conference with the theme of spread was held, where the seniors were supported to share their experiences with others and take the concept further. Used to evaluate this intervention were questionnaires, observations,interviews, personal stories and documentation from te project. Flow: Life café 1-reflection period (2-3 weeks) -Life café 2 and follow up- Test at home och making a Life Wheel (PDSA)-Life café 3 and follow up-Test at home and making a new Life Wheel- Life café 4 and so on......

Marketplace: Who else is addressing the problem outlined here? How does the proposed project differ from these approaches?

The important and main thing is to let the seniors see and understand the system around them. When they see how the different mechanisms act together, depend on each other, they get more knowledge and are more eager och motivated to change old habits to new habits and better lifestyle. You must understand WHY before you change for another habit. Example: My medicine is a very important theme for seniors. It is important not only to talk about the different pills. You must also as a senior know about food and drink, social activities, safety at home, physical exercises, oral health in order to change old habit to new habits. Otherwise the seniors go home and do as they always have done. Passion for life is built so you understand why, when and how . If you just focus on the pills you dont get a change in seniors habits. My medicine is part of all Life cafés today and one café is specially devoted to My medicine. Other organizations might address the same problem as we but we tackle the problem a little differenly. We want to get our seniors to go from words to action.

Qulturum-Center for Learning and Innovation in Healthcare, the improvement unit at Jönköping County Council located in the south of Sweden. The focus of Qulturum is to develop improvement knowledege concerning patients, patient involvement, design for heathcare and buildning prevental programmes. Qulturum is engaged in several improvement activities on a regional, national and international level

Please describe if and how your stakeholders (cooperation partners, funders, users, etc.) have been participating in defining the problem and developing the solution.

,Since Passion for life belongs to the Seniors themselves the users have had great chance to define the problems and possibilities. The Road map for instructors is continuously uppdated with the help of the seniors. This is very valuable. During 7 ys several seniors have helpt the programme to become better. Many telephone calls, letters, SMS, mails have reached us. The content in written instructions are updated by Qulturum in cooperation the the seniors. This has been of great importence because we often see that people want to do the programme their own way and then important factors can be missed. The road maps are written in Swedish and English.

Has your solution been tested in trials, experimentations, or pilot projects? If yes, please describe the process and outcome.

7½ ys ago the pilot started and after a year it ended in a programme. Passion for life
1.In the pilot a psychologist made personal deepinterviews with all participants.
2.Two ys after the pilot was finished a questionaire was sent to the 24 participants in the pilote group. 20 of 24 persons answered the questions
1. Passion for life has given me joy and encouragement in my everyday life
2. Would you recommend a friend to participate in a passion for life circle?
Results: 4 persons did not answer. 19 of 20 persons answered the questions with yes. Many comments were added describing how the seniors hade improved in their daily work.
3. In the personal improvement work with the Lifewheel analysis (PDSA) the seniors could follow personal progress over time. This was a very good tool to register and measure personal processes.

3. In the region of Skåne, southern part of Sweden, we have educated 500 circle leaders. After each eduction the participants get a written questionnaire evaluate the courses.
The results measured in the PDSA wheels point out for the individual how successful the process has been.

Earlier good experience of tackling barriers gives you courage to tackle new barriers. Barriers are to be overcome. The big senior experience help to overcome the barriers. Much support from Qulturum has been of utmost importance to overcome the barriers.

The fundamental base comes from the experience and competence of the seniors themselves.

Our team brings the pedagogic and medical competencies.

Passion for life started out as a pilote programme. From the beginning we emphasized that Passion for life belongs to the Seniors. It was not the administrators baby. After 1 ½ year it become a programme. A spread plan was early managed. and therefore there was no delay to go from pilote to programme. During these 7½ ys senior questions have come much in focus and this has been an unexpected och valuable event fro us. Very early we have had many visitors at our homepage which shows the growing focus on senior qustions during this time. Good economic planning is necessary.

Which target group(s) do you want to reach with your solution?

We want to reach seniors all ages globally.
If you want to reach the very ill elderly at different elderly homes and their relatives, there is an improvement potential to use this programme among the personal. IThe results could be a better daily life for the elderly.
To take part inPassion for life circles could be of a great value for the relatives of the sick elderly at elderly homes. It is no problem to change the programme for different the target groups.
We have also with good results tried this concept with elderly from other countries living in Sweden.

What is the impact on your target group (users, clients) you want to generate?

In Passion for life we want to generate empowered seniors who have learnt to use tools to live a good dailay life for the rest of their lifes. The good life style will help our seniors to use the hospital resourses when the really need them.

What is the wider impact on society you want to generate?

The wider impact is that enlightened and well experienced seniors will add a healthy life style to their life.
The seniors will be good examples for the younger generation. According to research are elderly happier than younger people. A suggestion is that younger people learn from elderly concerning structure of daily life. Elderly can learn much from the younger generation.
A next step in Passion for life can be to arrange Life cafés with Seniors and Juniors who learn from each other.

What are the impacts on your target group you already achieved?

In Passion for life where the elderly use the Life Wheel, the key stone in Passion for life, every senior can see and measure their own personal improvement. They make the improvements within different themes wich WHO recommend when working with elderly. This themes are safety at home, food and drink, social capital, physical training. To these four themes the seniors can add more themes. Much in focus for elderly today is My Medicine. A special café is dedicated this area, and questions on medicines are aswered att every café. Falling an be a lesser problem than today.
