Since 2011 the municipality of Sjöbo has been working with a project in cooperation with doctors at the Health Center and the hospital in the city of Ystad. The project concerns elderly people with a dementia diagnosis and how the parts cooperate to develope better support för the target group.

About Project

Problem: What problem is this project trying to address?

The problem is that only 75 % of the people with a diagnosis of dementia receives support from the municipality. The unrecorded is 25 % and these persons often lives under bad conditions in their homes and often get in contact with the municipality too late. These people and their relatives suffer and their health situation is often poor.

Solution: What is the proposed solution? Please be specific!

The team is working with a solution, a process, that means that when the elderly person gets a diagnosis of dementia they also receive information about what kind of support they can get from the municipality and who to contact. When the municipality meets the target group they ask what kind of support they and their relatives needs and therefore they are able to develop improvements measures like daily activities, support groups for relatives, and other individual support. The intention from the team is to work more preemptively. The team meets regularly and another purpose with the project is that the target group and their relatives only make one contact and then the team decides who is the best person in the team for future contact.
Impact: How does it Work

One example is the when the elderly person gets a dementia diagnosis the doctor informs what kind of support the municipality can offer and who to contact. The elderly person and their relatives receive a booklet. They contact the municipality or the doctor tells the municipality to contact the person or relative. After the contact there is a meeting and the elderly person and relative meets a social worker specialized in dementia and they together discuss what kind of support the elderly person and their relative needs. The social workers then is a long term contact for the target group and the relative and responsible for follow up needs and support. The social worker is part of the team and meets the doctors a couple times a year.

What makes a difference is that the target group and relatives can make decisions that are good for them and preemptive support from the municipality will be very important. The preemptive support also often increases the quality of life for both the target group and their relatives. Because of the early contact with the target group the municipalities, planning of long term support becomes more efficient and it also improves that the possibilities for the municipalities to work preemptive.

The process will include all health centres in the area and also hospital in the city of Ystad. Next step for the team is to study what kind of support is most frequently requested and also how the team can work to get more knowledge about the unrecorded.

There is only one solution to solve this issue and that is the way the team works. Both principals needs to cooperate around the target group and work with improvement measures to make the life quality better for the target group and their relatives. To be able to work preemptively its important to offer support at an early stage.
